Therapy Technical Instructor - Home First Team

Job overview

We are looking for a motivated and compassionate Band 4 Therapy Technical Instructor (TI) to join our Home First service.

This is an exciting opportunity to work as part of a dynamic, integrated multi‑disciplinary team supporting people to leave hospital safely and be assessed in their own homes, promoting independence and recovery while avoiding unnecessary hospital stays. The service operates across acute wards and community settings including Surrey and Sussex Healthcare, First Community Health and Care and Surrey County Council. The role is hosted by Surrey and Sussex Healthcare NHS Trust.

Main duties of the job

As a Band 4 Technical Instructor, you will:

  • Manage a delegated caseload of patients across hospital and community settings, under the direction and guidance of qualified therapy staff.
  • Undertake functional assessments, carry out treatment plans, and support safe discharge planning within the D2A pathway
  • Participate in  in‑reach assessments on acute wards and follow patients home to continue assessment and intervention in their own environment
  • Prescribe and issue basic equipment, complete environmental assessments, and support problem‑solving to maximise independence
  • Work closely across organisational boundaries with Occupational Therapists, Physiotherapists, Social Care, and wider MDT colleagues to deliver coordinated, person‑centred care
  • Contribute to service development, quality improvement, and the ongoing embedding of the Home First ethos.
