Pharmacist with an interest in Frailty required to cover Monday to Wednesday - 08:00- 16:00pm
An exciting opportunity has arisen for a pharmacist with a special interest in Frailty to join the Integrated Frailty Liaison Team at Wexham Park Hospital permanently.
This is a role in which you can develop your specialist pharmacy skillset and advance your clinical skills work working closely with the acute frailty Liaison Team.
As a Specialist Pharmacist, you will spend time in a patient-facing role enhancing your clinical skills. You will gain experience in treating a range of complex patients with multiple co-morbidities. You will work closely with the rest of the clinical pharmacy team, with access to a range of specialist pharmacists.
Frailty team is at the forefront of aligning with the national strategy for improving upon the existing processes involved in the identification of frailty and in the coordination of care between different Community and hospital teams to deliver better experiences and outcomes for our patients.
Main duties of the job
- The Frailty Service provides a front door assessment service including the development of comprehensive geriatric assessments and complex care plans to support admission avoidance, reduce length of stay and avoid unnecessary readmissions for individuals living with frailty.
- To provide high quality innovative clinical practice , be proactive in clinical decision making, underpinned by an advanced level of theoretical and practical knowledge, and be able to demonstrate improved patient care outcomes.
- Our service is 7 days a week and you will be part of the Pharmacy On-call service.

Qualifications
Essential criteria
- Pharmacy degree
- Non Medical Prescriber
- Diploma on Pharmacy
- Registered with GPhC
- Advanced physical assessment or willing to undertake
Desirable criteria
- MSc or higher degree
- Project management
- UKCPA membership
Experience
Essential criteria
- Experience of producing guidelines
- Experience of producing drug appraisals
- Experience of managing staff
- Significant post qualification experience working as a pharmacist
Desirable criteria
- Regular experience of providing clinical pharmacy support to Frailty.
- Clinical audit experience.
- Teaching/tutoring experience
Skills and knowledge
Essential criteria
- Excellent communication skills
- Ability to adapt and cope with challenges
- Applied clinical knowledge

- Excellent IT skills
- Evidence of interest in research
