The Heart Failure Specialist Nurse will be an integral member of the hospital-based multidisciplinary team, providing expert clinical knowledge and advanced nursing skills in the management of patients with a confirmed diagnosis of heart failure.
The post holder will manage a designated caseload of patients, with a particular focus on those with unplanned hospital admissions and individuals newly diagnosed with heart failure. They will deliver comprehensive assessment, evidence-based management, and ongoing follow-up care to optimise therapy and improve clinical outcomes.
Working collaboratively with the wider multidisciplinary team (MDT), the post holder will ensure effective communication and coordination of care across hospital and community settings. They will liaise closely with GPs, community nursing teams, and social care services to promote a seamless, patient-centred approach and to support patients and carers in developing self-management skills.
In addition, the post holder will contribute to the strategic development of the Heart Failure Service, working in line with National Service Framework (NSF) standards, NICE guidance, and local policies. They will be expected to influence nursing practice, participate in audit and service evaluation, and lead initiatives aimed at improving quality, safety, and outcomes for heart failure patients across the organisation.
Main duties of the job
Manage and facilitate the care of patients with a confirmed diagnosis of heart failure within the hospital setting.
Provide specialist assessment and coordinate individualised care plans to optimise health outcomes and reduce hospital admissions.
Utilise advanced clinical skills and evidence-based knowledge to identify patients’ needs, arrange appropriate investigations, and support treatment decisions.
Deliver expert medicines management support and advice in accordance with national and local heart failure guidelines.
Offer specialist advice, education, and emotional support to patients and their carers, promoting self-care and long-term condition management.
Liaise effectively with local cardiac rehabilitation and end-of-life care teams to ensure continuity of care across the patient pathway.
Act as a specialist resource for patients, carers, and healthcare professionals, supporting the development of knowledge and best practice in the management of heart failure.
