Are you an enthusiastic and motivated dietetic assistant practitioner looking for a new role that will challenge and develop you? The Pulmonary Rehabilitation multi-disciplinary team is a service supporting patients with chronic respiratory diseases out in the community. The purpose of the role is to support the registered dietitians in the education monitoring and support of patients and undertake related administration in accordance with the British Dietetic Association code of professional conduct for dietetic assistants and Code of Conduct for Health Care Support Workers in Wales. The post holder will be required to travel/undertake clinical work across the Swansea Bay UHB area to support service delivery working directly and indirevtly with patients and families in outpatient clinics/community groups and in their own homes.
Main duties of the job
Key Tasks and Responsibilities of the Post · To follow clinical protocols and manage own caseloads of clients referred to the Nutrition and Dietetic service · To undertake specific duties, without the Dietitian present which will include: o Carrying out initial interview and provision of appropriate nutritional support advice. o Encouraging and monitoring clients to follow dietetic care plan. o Carrying out nutrition risk assessment including weight, height BMI and percentage weight loss. o Calculation of nutrition requirements and dietary analysis. · To enable, encourage, support, supervise and assist clients who have had a treatment programme formulated by the dietitian. These clients will have a range of surgical and medical conditions and following clinical protocols (working under the direction of the dietitian) · To deliver of structured education programmes to groups of patients or health care professionals · To audit stock levels of ACBS products and supporting ward staff to manage stock control efficiently and effectively

Qualifications
Essential criteria
- Formal qualifications equivalent to HND/BTEC.
- Nutrition Qualification Agored Cymru level 3 or equivalent
- Willing to complete Agored Cymru Level 4 Assistant Practitioner Qualification
Desirable criteria
- Membership of the British Dietetic Association
- Teaching or structured education qualification
Knowledge
Essential criteria
- Nutritional knowledge including COPD, weight management and diabetes
- Behavourial change or motivational interviewing
- Knowledge of basic medical conditions and an understanding of health procedures relating to the work area.
Desirable criteria
- Structured Pulmonary Rehab, diabetes, weight management education
- General Health & Safety, Risk awareness, with the ability to relate this to individual circumstances and analyse actual risk and take appropriate actions.
Skills and Experience
Essential criteria
- Relevant experience of working in NHS or Social care setting
- Experience of delivering group education
- Good communication skills with staff and patient/clients
- Experience of team working
- Ability to keep accurate legible, patient/client and departmental records.
Desirable criteria
- Ability to travel within geographical area. Able to work hours flexibly.
