Are you a pharmacist ready for a new challenge and committed to enhancing your skills within a   primary care setting?

An excellent opportunity for an enthusiastic, adaptable and highly motivated pharmacists has arisen within the LLwchwr Cluster of Swansea Bay University Heath Board. This post will be varied providing your with the opportunity to work within a friendly and forward thinking team to improve the quality of prescribing, liaising closely with GPs, community based healthcare professionals and acute care colleagues at a time of transformational change within primary care.

Using excellent clinical knowledge, you will support the GP workforce within the cluster to deliver safe and effective patient care, through engagement, education and innovation. The successful candidate, ideally with independent prescribing status, will be ambitious with outstanding communication and organising skills and a proven ability to work independently.

Primary Care experience is desirable but we would welcome applications from those with hospital or community experience who are able to clearly demonstrate the aptitude required to work within a busy changing environment. There is one full time position available, however part time working and job share may be considered for the right candidate.

Main duties of the job

Support practices, clusters and the wider Health Board to improve the quality and effectiveness of medicines management within primary care in line with Prudent Healthcare principles.
•Reduce inappropriate poly-pharmacy and wasteful prescribing
•Responsible for developing, delivering and monitoring a range of primary care based pharmacy services
•Provide pharmaceutical care to a range of patients including those who may be frail and vulnerable and have complex needs
•Engage patients in their care using a patient focussed approach and co-production principles
•Utilise a holistic approach to support medicines optimisation in a variety of community settings
•Provide expert pharmaceutical advice, education and training to prescribers and other healthcare staff
•Develop close working relationships with a variety of health and social care professionals to maximise seamless care and ensure the best outcomes for patients
•Develop and extend the role, ensuring scope of practice is in line with competencies as defined within individual personal boundaries of clinical practice

Person specification

Essential Qualifications & Knowledge

Essential criteria
  • Degree in Pharmacy GPhC registration
  • Postgraduate Diploma in specialised area, other relevant qualification or equivalent experience
  • Evidence of recent and relevant Continuing Professional Development
  • Understands practical and emotional levers of prescribing behaviour
  • Understands non NHS organisations which impact prescribing
Desirable criteria
  • Registered with RPS
  • Member of or working towards (through foundation) RPS Faculty membership Independent prescriber status
  • Knowledge of the interface between health and social care
  • An understanding of budgets and financial information Working knowledge of prescribing software (CASPA) and GP in-practice clinical systems

Essential Experience

Essential criteria
  • Relevant post registration experience
  • Medication review skills Drug information experience
  • Demonstrate understanding of Primary Care systems, structures and culture
  • Experience of working within a healthcare team environment.
  • Previous experience of working with clinicians and multidisciplinary/multiagency work
  • Experience of facilitating change to improve clinical practice
  • Experience of delivering training sessions to varied groups
Desirable criteria
  • Experience in LHB/ primary care role People management experience
  • Evidence of strategy development and change management in complex situations
  • Development of services across the primary/ secondary care interface.
  • Expertise in Prescribing Analysis and Cost data Experience of project management
