An exciting opportunity has arisen for a dynamic, motivated individual to join Swansea Community Medicines Management Team. 

The post will be within the Living Well Service to provide support to individuals in the community setting. The post is ideal for an enthusiastic individual who would like join a friendly, vibrant and forward thinking team. It would suit someone interested in integrated working who is committed to developing and expanding their skills to achieve the best quality care and outcomes for patients in their own homes.

Through collaboration with a range of health and social care professionals, the post holder will work collaboratively with patients and others to promote self-management, early intervention and, where needed, improve support for the older person and those with frailty, along with involvement in discharge planning.

The role includes home visiting; providing interventions to individuals within their own home, involving lone working without direct supervision. You will liaise with a wide range of Health & Social Care professionals to identify solutions for individuals in a multidisciplinary network. Delivering training to relevant staff groups will also be included, and participation in audit processes.

Main duties of the job

The Living Well Service Navigator (CRT) will join an integrated, multidisciplinary community resource team to deliver support and education linked to care in domiciliary and intermediate settings. With an emphasis on self-management, early intervention and prevention for the older person, the post holder will contribute to improving patient outcomes, well-being and minimising harm, including avoiding unscheduled care, admissions, and delays to transfers of care. The role includes:

• Through collaboration with a range of health and social care professionals, the post holder will work collaboratively with patients and others to promote self-management, early intervention and, where needed, improve support for the older person and those with frailty, along with involvement in discharge planning.
• The post holder will participate in the safe and effective patient flow from secondary care to care of the CRT as well as supporting those in the community with Reablement needs in conjunction with the wider MDT
• To work under the direction and supervision of registrant staff in the provision of health care to individuals who meet the criteria for Medicines Management Team (MMT) within CRT in the community setting
