Job overview

The post holder will provide specialist nursing care, advice, guidance and leadership deploying their specialist theoretical and practical knowledge to;  

      Assess, plan, implement and evaluate evidenced-based nursing care.

      Provide specialist advice and excellent clinical practice.

      Work autonomously as a lone worker in the community.

      Provide specialist education to patients, their families and support new members of staff and students to develop their knowledge.

      Work collaboratively and co-operatively with clinical colleagues.

      Demonstrate the Health Board’s values and ensure that others demonstrate the same values.

 

Main duties of the job

      Work alongside the multi-disciplinary team and provide specialist nursing advice as appropriate to own areas of expertise. 

      Be visible and accessible in the clinical area to the nursing and wider multidisciplinary team, patients and service users. 

      Monitor and evaluate the standards of care provided.

Provide accurate, up-to-date, evidence-based information to patients and their families to ensure they make the best decision regarding modality.

This is a teaching post which involves travel within the regional area of South West Wales as a lone worker as per trust policy

Detailed job description and main responsibilities

Work autonomously as well as part of the wider MDT

As a regional centre you will be required to commute and lone work in the community covering the demographical area of South and West Wales

Provide education of all modalities and support patients and their families in making an informed decision

Be flexible in your approach to ensure patients receive information tailored to their needs in an open and honest environment where they can ask questions and speak freely to elevate their concerns

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply

Essential criteria
  • At least 2 years renal experience
  • Registered NMC nurse
Desirable criteria
  • Teaching qualification or relevant expierience
  • Renal course

  • • Specialist post graduate qualification or equivalent experience
