Job overview

This is an exciting opportunity for an experienced and motivated pharmacist to join the Intensive Care Unit (ICU) pharmacy team at Morriston Hospital.

You will provide advanced clinical pharmacy services to one of the busiest and most complex ICU in Wales, working alongside consultants, nurses and the wider multidisciplinary team to optimise medicines use and improve outcomes for critically ill patients.

The role offers the opportunity to develop expertise across a wide range of specialist areas including critical care pharmacotherapy, antimicrobial stewardship, medicines optimisation, therapeutic drug monitoring and independent prescribing.

You will become an integral member of the ICU multidisciplinary team, supporting patients through expert clinical review, evidence-based prescribing advice and safe medicines management from admission through to discharge.

As a Clinical Pharmacist, you will contribute to service development, governance and quality improvement whilst supporting the education and development of junior pharmacy staff.

This post is ideal for pharmacists looking to further develop advanced clinical practice within a supportive and innovative specialist environment.

Main duties of the job

·       Work collaboratively with pharmacy colleagues, consultants, nursing staff and the wider multidisciplinary team to deliver high-quality pharmaceutical care to critically ill patients.

·       Participate in daily consultant-led ward rounds, providing specialist medicines optimisation, prescribing support and clinical review.

·       Deliver a predominantly ward-based clinical pharmacy service, supporting patients from admission through to discharge through medicines reconciliation, therapeutic monitoring and discharge planning.

·       Provide proactive, evidence-based medicines advice to prescribers, supporting safe, effective and cost-efficient prescribing practice.

·       Deliver and support a range of medicines management services, ensuring the safe and effective use of medicines across ICU.

·       Identify opportunities to improve pharmaceutical services and contribute to the development and implementation of service improvements, governance initiatives and audit activity.

·       Provide clinical leadership and mentorship to rotational pharmacists, foundation trainees and pharmacy students through supervision, teaching and training.

·       Participate in the pharmacy dispensary and weekend rota, contributing to the wider pharmacy service as required.
