Job overview

An exciting opportunity has arisen for an enthusiastic, dynamic Band 6 District Nurse to join our Regional Single Point of Access Service.

We have  a 22.5 hour position, hours to be worked over a variety of shift patterns.

The post holder will triage urgent, non-urgent and timed visits and will use professional judgment to determine appropriate nursing care, initiating and co-ordinating input from other services as required.

The post holder will be a motivated, reflexive practitioner and will be a key member of the District Nursing Team, working closely with the  District Nursing Team in the provision of high quality, holistic care at the point of referral. 

Applicants must hold the Diploma or BSc in Community Health studies or equivalent, be a nurse prescriber and have a commitment to continuous professional development. 

 

Main duties of the job

The successful applicant must have a flexible approach to the needs of the service and be motivated and enthusiastic in order to embrace the changes required to improve patient outcomes and service delivery.

The successful applicant will possess well developed leadership and management skills, with the ability to motivate individuals and the team.

You will need to be an excellent communicator with sound clinical skills and the drive and determination to contribute to the delivery of high quality nursing care and outcomes.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
