Do you want to work in a team where you can shape someone’s first experience of mental health services, share your opinions, knowledge and expertise and make a difference, use evidence based interventions and be involved in research, receive training and awareness in interventions we know work with the client group?

If this is you then the North Tyneside Early Intervention in Psychosis (EIP) Team would like you to join us.   

The post is a Band 6 Community Practitioner role working across Northumberland, offering interventions to 14 – 65 year olds who are thought to be developing a first episode of psychosis.

You will work as part of a Multi disciplinary Team (MDT), be involved in assessing and providing interventions to service users as well as supporting families, whilst also working collaboratively with other professionals.  The service is committed to developing the skills of practitioners, so there will be opportunities to develop further clinical skills. You will be involved in care co-ordination, but with a  smaller caseload allowing greater opportunity to offer interventions / treatment.

Are you a Mental Health Nurse (RMN), Learning Disability Nurse, Occupational Therapist or Social worker? Do you have a passion to support service users in their recovery, engage and involve families in care planning and in using formulation to help a service user understand experiences?

In the role as a community practitioner you will:

- triage referrals and allocate for assessment where appropriate.

- undertake initial assessment, feedback to MDT.

-  act as point of contact for referrals and calls where they are not able to wait.

- manage a caseload, complete risk assessment and care  planning, liaise with other professionals within and external to the team .

-  provide evidence based interventions or support plans.

Clinical Supervision and  development of skills necessary for the role are offered within the team and you will be supported to develop.

As part of the Early Intervention  

  • To be responsible for the Assessment and Management of casework of service users who have substantial and complex needs.
  • Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, 3rd sector partner agencies and the public.
  • In collaboration with the Service User and Carer where appropriate, develop Recovery and Wellbeing focused Care Plans, ensuring regular reviews to monitor progress and adapt Care Plan as necessary.
  • To consider Carer needs and ensure Carer and family are involved in Care

Planning where appropriate

  • To deliver evidenced based Clinical and Psychologically based Interventions.
  • The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.
  • Coordinate Treatment Planning & Interventions, Reviews and Discharge

Planning.

  • Provide clinical supervision to staff as delegated by Pathway Clinical Lead and Pathway Manager.
  • To provide compassionate care that is based on empathy, kindness, respect and dignity.
  • To incorporate a client centred philosophy into nursing practice and client centred collaborative care.
  • Work flexibly to meet the needs of Service Users across pathways which may regularly include working extended and flexible hours over 7 days a week.
  • Demonstrate flexibility in the role. This may involve working across clinical pathways to support the needs of the service, whilst ensuring the delivery of high quality care at all times.

Qualifications

  • Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
  • Mentor Training.
  • Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
  • Proven track record of post registration clinical experience relevant to the post.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
  • Up-to-date knowledge/experience of relevant legislation.

Experience

  • Has an understanding of First Episode Psychosis and its function.
  • Has worked with families and carers
  • 12 to 24 month band 5 qualified nurse experience dependent upon service need.
  • Awareness and understanding of professional accountability, NMC standards and other local and national initiatives.

  • Holds a CBT or Family intervention qualification

Skills

  • Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs

  • Demonstrate an understanding of effective & appropriate treatment options including a range of evidence based clinical approaches as identified in N.I.C.E. Guidance).
  • Demonstrate effective written and verbal communication skills.
