BAND  5  - Community Staff Nurse

We wish to recruit a band 5 Community Staff Nurse to join a Locality Team in Gateshead. The locality teams are multi professional working in collaboration to ensure safe, highly effective patient care.

This post will provide an exciting opportunity to deliver a high quality service to patients in Gateshead.

You will need to be motivated, organised and flexible. The service hours are 8am – 8pm, 7 days per week. You will be expected to work flexibly to deliver service needs.

You must be a registered nurse.  Post registration experience and ongoing professional development are desirable.

It would also be desirable to have evidence of experience in a community setting.

Full time and 24 hour post available 

Main duties of the job

The delivery of high standards of clinical practice that hold patients and their families at the center to ensure that they receive safe, quality care that gives them a good experience.
To be responsible for delivery of an identified package of care under the supervision and direction of Specialist Nurse Practitioner/District Nurse/Locality Team Lead.

Detailed job description and main responsibilities

  • To provide high quality patient care in the community setting under the direction and supervision of a senior nurse/practitioner.
  •  To provide hands on care to patients in a variety of settings within the community
  • To maintain standards of care through evaluating and implementing evidence based practice
  • To delegate work appropriately according to skill mix ensuring maintenance of  safety within the community.
  • To support the development of other staff and the service provided.
  • To support the assessment of the health and health related needs of patients, clients, their families and other carers.
  •  To treat specific diseases in accordance with agreed medical protocols under direction of the senior practitioner.
  • To assess, plan, provide and evaluate nursing care to meet care needs of individual patients and carers in their own homes.
  •  To analyse and evaluate personal clinical performance in a reflective manner to ensure continued personal and professional development and the maintenance of an up-to-date knowledge base.
  • To work with Trust policies and procedures that ensure patient safety at all times.
  •  To work independently under direction within a multi-disciplinary of an integrated workforce that is based in the community and is designed to meet the health needs of the identified locality.
