Are you a motivated, enthusiastic, and experienced Registered Nurse seeking a change of direction? An opportunity has arisen within NHS North East and North Cumbria Integrated Care Board (NENC ICB) to join the All-Age Continuing Care team (AACC) as a Continuing Health Care (CHC) Assessor to deliver an effective assessment, review, and case management service in the Tees Valley area.

Main duties of the job
The successful applicant will:
- Demonstrate knowledge and skills in completion of person-centred assessments to aid decision making for NHS funded care, working within a multi-agency environment.
- Ensure the health and social care needs of people referred for Continuing Health Care (CHC) eligibility are identified and met in accordance with the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care (FNC)
- Collate information for and complete comprehensive and co-ordinated assessments for NHS funded care with clients, carers and colleagues within health and social care
- Commission services to meet the needs of eligible clients
- Case manage CHC clients ensuring timely reviews are conducted to ensure health and care needs are being met appropriately.
- Ascertain care packages continue to be effectively commissioned and are cost effective in line with National Policy.
- Promote Personal Health Budgets (PHBs) ensuring integration into Continuing Health Care and offer this where requested to those clients who fulfil agreed criteria.

Role requirements:
- Current registration with the Nursing & Midwifery Council (NMC)
- Relevant experience of working in a complex organisation – NHS or Social Care.
- Experience of working in partnership with the wider health and social care team.
- Experience of working with vulnerable people and their families/carers.
- Demonstrate ability to work to defined targets and deadlines.
- Highly developed interpersonal skills and communication skills.
- Analytical skills.

- Evidence of working within a Multi Disciplinary Team
- Registered Nurse on NMC register
- Evidence of degree level study or equivalent x

- Training qualification/ENS 998 or equivalent

- Experience of working within CHC arena and specifically Appeals and retrospective service
- Community nursing experience
- Experience of Independent Sector Nursing
