Job overview
Full time - 37.5 hours per week
The Clinical Risk and Investigation Department are looking for a Dynamic, motivated and passionate candidate to join the team in the role of full time investigating Officer (Clinical Incident Lead).
The purpose of the role is to investigate any patient safety incidents that have been identified within Cumbria Northumberland, Tyne and Wear NHS Foundation Trust. You will be required to work with clinicians and managers to identify significant risk across both clinical and non-clinical areas.
You will ensure families and or Service users are involved and supported in the process so that they are part of the learning review where ever possible, taking into consideration consent to share complexities.
The post holder will ensure that the family’s needs are met with regard to the provision of timely and accurate information during the investigation, openness and transparency and to reflect the level of involvement they wish to have in the investigation.
You will arrange and chair after action review meetings with all significant and relevant parties as well as utilising other SEIPS style tools to review an incident.
Main duties of the job
You will be required to produce and deliver a Patient safety Incident report. This will be presented to the Patient Safety Learning and Improvement as well as assurance meetings within the Care Groups. This requires accuracy and the ability to communicate highly complex and sensitive information. The role requires the highest degree of sensitivity, communication and integrity.
The post holder will also be expected to monitor incident activity from service areas and ensure data quality standards. You will be expected to support the development of live dashboards relating to incident activity to aid learning as well as supporting the development of the risk management system to ensure learning across the organisation. Where directed you will conduct analysis and investigations to produce and present thematic reviews of complex incident data/activity that allows services to implement targeted improvement strategies.
The Post requires a clinical registration and you should have experience of collaborative working across statutory and non statutory agencies and have experience in a senior clinical and or managerial role. You will require an understanding of the Patient Safety Incident Response Framework and how CNTW work within this Guidance.

Education and Qualification
Essential criteria
- To be educated to Masters level or a relevant clinical / professional qualification.
- Five years post qualification experience.
- Excellent management / leadership skills.
- Evidence of continuing professional or managerial development.
Desirable criteria
- Teaching qualification
- Root Cause Analysis training
- Human Factors training
Knowledge and Experience
Essential criteria
- Interpretation and implementation of national strategies into local initiatives.
- Ability to provide specialised advice to clinical and non-clinical areas on service development and re-design initiatives in line with the Trust’s business
- A good working knowledge of the requirements of clinical service delivery in Inpatient, Community and Specialist services.
- This will include the performance management agenda in the NHS and clinical governance, and the needs of service users across the Trust.
- Good understanding and working knowledge of the Serious Incident Framework (March 2015)
- Experience of providing an overview of findings in a court setting
Desirable criteria
- A thorough understanding of the NHS Modernisation Programme.
- Knowledge and experience on NHS business planned process.
- Thorough working knowledge of the requirements of the Inpatient, Community and Specialist services including performance management within the NHS, integrated governance and the needs of all service users and carers

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
