Job overview

This is a substantive post in paediatric haematology with responsibilities in caring for children undergoing Haematopoetic stem cell transplant and CAR-T treatment as well as those with leukaemia. The candidate must be able to evidence relevant expertise and training. The post is open to those who hold FRCPath (or equivalent) and have relevant paediatric experience, and to candidates who have trained as paediatricians and can demonstrate relevant training in haematology, bone marrow transplantation and cellular therapies. 

The role will be in stem cell transplantation, cellular therapies and leukaemia primarily, and will include on call cover for the paediatric stem cell transplant programme.

Candidates for the post must be registered medical practitioners and on, or eligible to join, the Specialist Register within six months of the date of the Appointments Committee.

Main duties of the job

This post is based in the paediatric allogeneic stem cell transplantation and advanced cellular therapies program. It will provide an additional consultant post overseeing haematological transplants (malignant and non-malignant) and work alongside the supra-regional immunodeficiency transplant team. It will also include a role in Advanced Cellular Therapies Service delivering CAR-T therapies, as well as leukaemia work.

Detailed job description and main responsibilities

Allogeneic Stem Cell Transplant:
•    Shared management of paediatric stem cell transplant patients with haematological disorders including malignant and non-malignant indications. Responsible for all aspects of the transplant journey including inpatient and outpatient care.
•    Shared management of paediatric transplant inpatients with non-haematological indications.

Advanced Cellular Therapies (CAR-T):
•    Expand the consultant provision overseeing the CAR-T service. Responsible for patient work up and treatment, outpatient management and liaison with base hospitals. This would include being part of the national CAR-T panel
•    The Great North Children’s Hospital is one of 3 UK centres commissioned for CAR-T therapies for children and young people (up to 18 years of age). As well as licenced products, we have early phase trial products available and we are well placed to develop CAR-T and other cellular therapies for other indications in the future.

Malignant Haematology:
•    Clinical care of patients treated at the Primary Treatment Centre at GNCH. This would include the management of patients with leukaemia and malignant haematological conditions.

Laboratory Duties:
•    Suitably qualified (FRCPath or equivalent) individuals would be expected to contribute to the paediatric haematology laboratory service including reporting of blood films, bone marrows and CSF specimens as well as contribution to liaison laboratory haematology with other paediatric subspecialties and general paediatrics.
•    Further responsibilities include the provision of bone marrow examinations, lumbar punctures and delivery of intrathecal chemotherapy within the painful procedures lists. It will also include bone marrow harvests.
•    It is anticipated that the new appointment will enhance the already close interaction between the Departments of Haematology, Paediatric and Adolescent Oncology and Immunology.

On-call Duties:
•    There is a ward attending system during working hours and an out of hours on-call rota (1 in 5) within the integrated stem cell transplant service (including immunodeficiency) based on ward 3.  
•    Oversight and supervision of inpatients undergoing cellular therapies
•    Out of hours consultant haematology laboratory and Haemostasis and Thrombosis cover is provided by sub-groups of the adult haematologist service. 
•    Out of hours cover for leukaemia and CAR-T patients is provided by the consultants covering the paediatric haematology and oncology service
•    Leave cover will be provided for the post holder by paediatric immunology and haematology colleagues. Appropriate separate secretarial support will be provided for clinical and laboratory activity.  

Administrative: 
•    The appointee will be expected to take a normal role in management, administration and audit within paediatric haematology/oncology, paediatric transplantation/cellular therapies and clinical and laboratory haematology.  

Research:
•    The appointee’s help and involvement in current research within paediatric haematology, transplantation and cellular therapies will be welcomed, and the development and extension of his/her own research fields will be encouraged.
•    The department is actively involved in early phase and NCRI badged trials. As a European Cancer Medicine Centre we have been formally designated by the Innovative Therapies for Children with Cancer and European Early Phase Clinical Trials Consortium as a First-In–Child Study Centre.  

Teaching & Education:
•    The appointee will be expected to contribute to undergraduate teaching, more focused paediatric haematology post-graduate teaching and to the higher specialist training of specialist registrars in haematology as an educational supervisor and paediatric trainees as clinical supervisor. 

Person specification

Education, Qualifications and Specialist Skills

Essential criteria
  • Primary Medical Qualification
  • Full GMC Registration
  • Entry onto the GMC Specialist Register (within 6 months from the date of the AAC)
  • Special skills and training in Paediatric Haematology/HSCT
  • A minimum of 12 months’ experience in paediatric haematology (applicants without MRCPCH are recommended to have a minimum of 2 years paediatric haematology experience)
  • Minimum of 12 months’ experience in general paediatrics
Desirable criteria
  • Higher Degree
  • FRCPath or equivalent

Clinical Experience

Essential criteria
  • Experience in preparation of children for haematopoietic stem cell transplantation (HSCT) and of their care during and after HSCT
  • Specific experience and interest in Paediatric Haematology
  • Paediatric Haematology/HSCT training in a recognised training centre
  • Ability to take independent responsibility for the clinical care of children with haematological illness referred to a tertiary centre.
  • Ability to offer an expert clinical opinion in children with haematological illness
Desirable criteria
  • Experience in preparation of children for CAR-T therapy and of their care during and after CAR-T
  • Wide experience of clinical and laboratory haematology

Management and Administration Experience
