An exciting opportunity has arisen for a Band 7 Nurse for NHS North East & North Cumbria ICB.  You will be part of the All Age Continuing Care team (AACC) (adults) and be committed to high quality patient and family experiences, working alongside other partners and stakeholders to ensure that patients receive safe, caring, responsive and well - led care to meet their needs.

The right candidate will have a high standard of leadership, communication and organisational skills and have experience and/knowledge of Continuing Healthcare. The successful applicant will oversee the complex components of Continuing Healthcare and NHS Funded Care. They will lead, motivate and support the Nurse Coordinators ensuring quality service outcomes are delivered.  The successful applicant will lead a continuously improving service, by ensuring systems are in place to support clinical governance, quality monitoring and ongoing research and audit of practice which will improve the patient/client journey.

Our Integrated Care Board (ICB) is the largest in the country. 

The ICB is responsible for ensuring that high quality and safe health services are accessible to all of our communities. It has a wide range of functions, including; promoting integration of health and care services, improving peoples health and well-being, reducing health inequalities.

With a £6.6 billion budget and a workforce of 170,000 people across the North East and North Cumbria. As well as strategic functions, ICB staff also work at place level with local health and wellbeing boards in each of our 14 local authority areas. These teams also work alongside our 67 primary care networks which are groups of local GP practices, social care teams and other community- based area providers.

The ICB works with our Provider Collaborative too, this includes the 11 NHS Foundation Trusts in the region, to deliver our shared priorities.

By joining our team you will be helping us to achieve our key priorities over the next year:

  • To continue to complete AACC work within specific timeframes set within the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care and make ongoing improvements to the process achievement of National targets which include number of referrals completed within 28 days and assessment conversion rates.
  • Use tools available (National Quarterly data, CHAT, audit) to review and manage risks within the CHC team, relating with corporate risks which will impact on the wider risk to services and the community.
  • Develop a specific AACC workforce plan at place to include a full system and team review streamlining services so that they are in line with other AACC Teams in the ICB and work with the ICB to deliver the wider ICB AACC workforce plan recognising speciality and function with function with focus on individual development and succession planning. 
  • Focus on consistency of the application of the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care framework and align audit processes from referral to end.
