Trainee Vascular Access Practitioner

Job overview

The Practitioner Work as part of a specialist team, will be responsible for the clinical examination and assessment of patients in order to select and insert appropriate venous access devices for patients requiring intermediate and long term intravenous therapies throughout the UHCW trust. Such therapies will include intravenous nutrition, chemotherapy and long-term antibiotics

Banding will be based on experience in Vascular access.

Main duties of the job

  • To insert and remove medium/long-term VADs such as midlines, PICC, Hickman and Portacaths
  • Follow-up of patients post line insertion and supporting clinical areas in the management of all longer term vascular access devices (VAD)
  •  Assess, plan, implement and evaluate programmes of evidenced based nursing care in relation to VAD management and acces
  •  To provide specialist advice and work closely with all members of the multi professional teams throughout UHCW NHS Trust in relation to VAD management and access
  •  To review and update the Trust VAD protocols
  • To review and coordinate, or where necessary establish appropriate training is in place for all healthcare professionals involved in VAD management
  • To establish a process of ongoing competency testing for healthcare professionals involved in VAD management
  •  To provide specialist support to nurses, healthcare professionals, other trust staff, patients and visitors in relation to VAD management
  •   Participation in audit and surveillance of service delivery

 

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP

Detailed job description and main responsibilities

  • Assessment, prioritisation and scheduling of patients from in-patient wards and day case waiting list for the insertion of vascular access device
  • To insert and remove medium/long-term VADs such as midlines, PICC, Hickman and Portacaths
  • Follow-up of patients post line insertion and supporting clinical areas in the management of all longer term vascular access devices (VAD)
  • Assess, plan, implement and evaluate programmes of evidenced based nursing care in relation to VAD management and access

Clinical Responsibilities:

  • To insert and remove medium/long term VADs such as midlines, PICC, Hickman and Portacaths
  • Perform comprehensive assessment of patient nursing needs, plan, implement and evaluate care delivery according to changing health care needs in relation to VAD management and access.
  • Perform advanced clinical skills in relation to VAD management and access
  •  Collect, collate, evaluate and report information, maintaining accurate patient records. Involve patients and carers/relatives in the planning and delivery of care and development of services
  • Work collaboratively with other professionals and agencies to ensure patient needs are met, especially in relation to ongoing care needs and discharge arrangements.
  • Establish and maintain effective communication with patients and carers/relatives, and professional across health and social services
  •   Develop knowledge and practice in an area of work regarding VAD access and placement and management. This will include the use of specialist equipment (e.g. ultrasound)
  •   Participation in a local and Trust wide audit process for VAD management
  •   To assist with research and development
  •    Contribute to the development of VAD services including placement (including placement under ultrasound), education, competency testing and audit
  • Contribute to quality improvement around VAD management.
  • Promote people’s equality, diversity and rights
  • To organise own specialist workload
  • To prioritise workload of themselves and others (as required), assess performance and provide clear constructive feedback to team members
  •   To be empathetic and reassuring when communicating highly sensitive condition related information and advice to patients, carers and relative
