We are seeking a compassionate & experienced Mental Health Nurse to join our evolving team of placed based homeless workers. This unique & bespoke role, In partnership with CGL offers the opportunity to deliver trauma-informed, person-centered care to individuals facing homelessness, mental health challenges, & substance misuse.
You will work closely with the Coventry Clinical Lead & CGL to bridge gaps in service provision & improve outcomes for this highly marginalized population who face significant health inequalities & barriers to care. This is a patient facing community role.
Main duties of the job
Key Responsibilities
· Provide mental health support to adults who are rough sleeping, homeless, or at risk of homelessness.
· Facilitate access to drug/alcohol services/support & mental health services.
· Collaborate with voluntary & statutory services, including physical health & outreach teams.
· Jointly assess & support individuals with dual diagnosis in partnership with CGL.
· Work flexibly & autonomously, this may include on occasions outside st&ard hours, to meet client needs.
· Advocating for service users, reducing disengagement, & helping them navigate services.
· Providing responsive clinical input, including crisis management & harm reduction in community settings.
· Actively contributing to multi-disciplinary team (MDT) meetings to ensure joined-up care.

Requirements
· RMN with post-registration experience in community mental health.
· Substantial experience working with adults & young people, preferably in community settings.
· Ability to travel during the working day (car driver essential).
