Job overview
We are looking for a highly motivated and enthusiastic nurse with proven specialist knowledge and experience in cardiology to join our established multidisciplinary Cardiac Rehabilitation Team at University Hospitals Coventry and Warwickshire (UHCW).
Cardiac rehabilitation at UHCW is delivered across two community sites – the Centre for Exercise Health, Coventry and Rugby St Cross Hospital. The successful post holder will manage a defined caseload across both sites, working to a clear and structured work plan to ensure safe, effective, and patient-centred care.
You will be joining a large and supportive multidisciplinary team and will work alongside other experienced cardiac rehabilitation specialist nurses to collectively manage the heart failure cardiac rehabilitation caseload.
You will work closely with cardiology, heart failure, and community teams, contributing to the delivery and ongoing development of a high-quality cardiac rehabilitation service aligned with national guidance and standards.
This is an exciting opportunity for a nurse who is passionate about improving outcomes for cardiac patients and delivering holistic, evidence-based care within a supportive and forward-thinking team.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
Main duties of the job
A key component of the role will be the management of a dedicated heart failure caseload, supporting patients following diagnosis or hospital admission referred for cardiac rehabilitation. This will include undertaking comprehensive assessments, delivering evidence-based interventions, supporting medication optimisation alongside the wider MDT, and providing education to enable patients to self-manage their condition and reduce the risk of deterioration and readmission.

Detailed job description and main responsibilities
- To work autonomously taking responsibility for care, quality and clinical activity for the heart failure cardiac rehabilitation caseload, including patients attending a programme of cardiac rehabilitation across inpatient and community pathways, working in collaboration with hospital and community heart failure teams.
- To process heart failure cardiac rehabilitation referrals appropriately, ensuring that patients are reviewed within designated waiting times and enrolled into a cardiac rehabilitation programme, with tasks delegated to administration staff effectively and efficiently and communicated to the wider multidisciplinary team, including hospital and community heart failure services.
- To triage and prioritise heart failure patients attending cardiac rehabilitation, demonstrating the use of a variety of techniques to elicit a detailed clinical history, assess symptom burden, and determine an appropriate evidence-based care plan in collaboration with heart failure teams.
- To take responsibility for direct patient care: assessing, planning, providing and evaluating nursing care needs for heart failure patients attending a cardiac rehabilitation programme, including monitoring, diagnostics, and investigations relevant to the condition, liaising closely with hospital and community heart failure teams.
- To facilitate appropriate clinical interventions for heart failure patients within the cardiac rehabilitation programme, including supporting optimisation of medical therapy, interpreting and acting on results, working within local and national guidelines, and referring to or working jointly with hospital and community heart failure teams or general practitioners where necessary.
- To undertake interventions consistent with evidence-based heart failure and cardiac rehabilitation practice, supporting patients participating in rehabilitation programmes, and transferring and applying knowledge and skills with wider multidisciplinary team members, including heart failure services, to meet patient needs.
- To evaluate the effectiveness of interventions in achieving agreed heart failure management and cardiac rehabilitation goals, working collaboratively with hospital and community heart failure teams and making any required changes to optimise outcomes.
- To collaborate with heart failure patients attending cardiac rehabilitation programmes and their carers, promoting patient activation, health education, and self-management, enabling individuals to understand their condition, recognise deterioration, make informed decisions, and confidently manage their long-term health.
