An exciting opportunity has opened for an ambitious, dynamic and enthusiastic Social Prescriber  to join the cardiovascular rehabilitation team at UHCW NHS Trust. Funding is available to conduct an 18 month pilot examining the impact of an embedded social prescriber on the experience of those invited to attend cardiovascular rehabilitation. This is a fantastic and long anticipated opportunity for the service to explore the impact this role has on the experience of those offered cardiovascular rehabilitation.

People's ability to attend cardiovascular rehabilitation is often influenced by complex social factors that the traditional cardiac rehabilitation MDTs are unable to address. It is hoped that the successful candidate, working in tandem with members of an established cardiac social prescribing network will support people in addressing issues relating to wider determinants of health such as social deprivation and isolation and support their access to rehabilitation or suitable alternatives that promote a healthier lifestyle.

This is a newly created fixed term (18 months) position, introduced to explore the impact of an embedded social prescriber role on the recruitment and retention of patients to a community cardiac rehabilitation programme.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.

The primary purpose of this role will be to contact all patients who decline, drop out or are struggling to participate in our cardiovascular rehabilitation programme. The successful candidate will offer such people an opportunity to explore potential obstacles & support individualised solutions that create conditions that may lead to higher levels of engagement in rehabilitation & health promoting lifestyle modification.

  • Provide personalised support to individuals, their families & carers to take control of their wellbeing, live independently & improve their health outcomes. 
  • Take referrals from members of the multi-disciplinary (nurses, clinical exercise physiologists, dietitian, pharmacist, Talking Therapies) cardiovascular rehabilitation team.
  • Be proactive in encouraging self-referrals & connecting with all local communities, particularly those communities that statutory agencies may find hard to reach. 
  • To contact all patients who have declined or dropped out of cardiac rehabilitation & offer support in overcoming obstacles to participation and/or discuss potential alternative health promoting options available in across Coventry & Warwickshire

This is a fantastic and long anticipated opportunity for the service to explore the impact this role has on the experience of those offered cardiovascular rehabilitation.

We are seeking an experienced social prescriber with excellent communication skills and a passion for improving people's access to health enhancing lifestyle choices. This role is well suited to individuals with experience of managing a caseload of clients, working one-to-one with clients, families, and carers to plan and help people resolve practical issues such as housing, finances, employment that will enable health supporting activity and social connection.

The role will be primarily based at our community cardiovascular rehabilitation service in Coventry city centre, with the potential to provide cover at our hospital-based service in Rugby, as service needs are identified.

  • Develop trusting relationships by giving people time and focus on ‘what matters to me’. Take a holistic approach, based on the person’s priorities and the wider determinants of health. Co-produce a personalised support plan to improve health and wellbeing, introducing or reconnecting people to community groups and statutory services.
  • The role will require managing and prioritising your own caseload, in accordance with the needs, priorities and any urgent support required by individuals on the caseload 
  • It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when what the persons needs is beyond the scope of the link worker role – e.g., when there is a mental health need requiring a qualified practitioner. 
  • Draw on and increase the strengths and capacities of local communities, enabling local VCSE organisations and community groups to receive social prescribing referrals. Ensure they are supported, have basic safeguarding processes for vulnerable individuals and can provide opportunities for the person to develop friendships, a sense of belonging, and build knowledge, skills and confidence
  • To establish and maintain good connections with internal and local social prescribing networks that support your ability to deliver appropriate onward referral for issues identified in your consultations.
