University Hospitals Coventry and Warwickshire NHS Trust is seeking to recruit a Band 3 Community Health Support Worker to join our Community Services team.

This post offers an excellent opportunity to work within a city-wide community service, supporting adults with a range of health and wellbeing needs to maintain dignity, independence, and quality of life in their own homes and local communities.

Secondment will be considered for internal candidates – If you are successful after interview, you must obtain approval from your current line manager to accept the offer.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP. 

Main duties of the job

  • To demonstrate effective communication and interpersonal skills to support patients in managing their health and wellbeing. The post holder will assist in the delivery of care and support to a defined caseload of patients under the direction of a registered nurse or senior clinician. There may be complex or challenging situations to manage, including refusal of care, disengagement from services, or patients presenting with increased risks to themselves or others, with support from senior staff where appropriate.
  • To communicate accurate and sensitive information to patients, carers, and relatives using an appropriate and accessible approach, taking into consideration factors such as background, culture, gender, age, and any physical, sensory, or learning difficulties. Communication may be sensitive and should be adapted to ensure effective understanding and reassurance.
  • To respect the dignity, privacy, independence, and preferences of patients at all times and support the delivery of person-centred care in line with care plans and Trust policies.

Detailed job description and main responsibilities

  • Provide practical, emotional, and social support to patients and carers while maintaining confidentiality and promoting independence and wellbeing.
  • To assist patients with activities of daily living including personal care, nutrition, continence care, mobility, pressure area care, and monitoring of basic observations in line with delegated responsibilities and training.
  • To undertake delegated clinical duties including simple dressings, and administration of simple medicines following Trust policies, procedures, and competency-based training.
  • To recognise and report changes in patients’ symptoms, behaviours, or conditions to registered health care professionals, including signs of deterioration or end of life care needs.
  • To liaise with members of the multidisciplinary team, local services, carers, and other agencies to support the seamless delivery and transfer of patient care within the pathway.
  • The post holder will be required to manage and prioritise their own workload efficiently to meet the needs of the service within a context of competing demands, fluctuating patient needs, and varying levels of risk.
  • To support discharge planning and ongoing care by encouraging patients, carers, and family members to participate in informed decisions regarding future care and support needs.

The group is currently trialling a new Integrated Neighbourhood Team (INT) model following a collaborative event with internal and external stakeholders. This pilot aims to explore team structures and assess the impact on patient experience. While no formal redesign has taken place yet, the outcomes of this trial may inform future service delivery and team configuration across the group.

If you are successful in your application, please be aware that your team structure and line management may be subject to change in the future as part of this evolving model. However, any formal changes will be preceded by appropriate consultation in line with organisational policy.
