Job overview
Coventry and Warwickshire Partnership Trust are looking for an enthusiastic and self-motivated individual to join our RISE Services as an admin Patient Flow Co-ordinator on a full-time basis. This role will be based within Intervention Pathway Hub Team, supporting the Specialist Mental Health Service.
The post holder will have excellent organisational skills, effective communication skills and the ability to be innovative in regards to the way in which clinical assessments and interventions are co-ordinated.
This role will be office based within The Abbey RISE Center and will also require travel to other bases across Coventry and Warwickshire on a regular basis.
Main duties of the job
The main responsibilities of this post are to co-ordinate the delivery of clinical interventions for children and young people and their Families within the RISE Service. You will support specific processes for data management to contribute to the development of services, improve clinical outcomes and contribute to clinical and service audits.
Main Duties:
- To liaise with other members of the multi-disciplinary/multi-agency team, completing relevant forms and documentation to support referrals to the Specialist Mental Health Team with the required degree of urgency under the supervision of the Patient Flow Manager.
- Co-ordinate appointments .
- To support multi-disciplinary team meetings by completing and outcoming agendas, adding clinical notes of conversations to patient records and supporting patient flow onto the treatment waiting list.
- To support patient flow with regards to offered interventions.
- To ensure that up to date written and/or electronic records and activity data are maintained in accordance with professional and Trust standards and policies.
- To participate in the signposting process, contacting appropriate resources and/or support in order to facilitate effective discharge.

Qualifications
Essential criteria
- English and Maths GCSE grades A-C or or equivalent qualification/ experience/ willing to work towards completion of this.
- Commitment to lifelong learning and continuing personal and career development.

- QCF Business Administration Level 3
