Main duties of the job

  • To review all patient pathways day 55 onwards on a daily basis.
  • Chase on a daily basis appointments for procedures/outpatients/inpatient admissions for patients to escalate to the MDTC Team leader/ Cancer Services Manager/Lead Nurse any delays to the patients pathway or issues where delays may occur in the absence of the site specific MDT Co-ordinator.
  • Schedule and co-ordinate data for regular MDT tumour site meetings. This includes preparation of patient schedule lists together with their health records, associated diagnostic results and information provided by clinician.
  • Prepare patient agenda’s for discussion at MDT meetings, using information supplied by the Pathology Department, Clinicians and Clinical Nurse Specialists.
  • Circulation of agenda’s to all Core members of the MDT prior to meetings and facilitating the addition of urgent inclusions.
  • Responsible for collating all pathology material in liaison with the relevant site lead pathologist and providing reports in preparation for meetings.
  • Responsible for collating all radiologist material in liaison with the relevant site lead radiologist in preparation for meeting.
  • Ensuring the availability of all supporting material across the disciplines to enhance the patient’s treatment/management plan, ie Patient’s notes, Pathology, radiology reports including X- Rays and scans.
  • Attend meetings and record attendance.

Detailed job description and main responsibilities

To track patients pathways on a daily basis (focus on day 55 onwards and diagnosed patients particularly)

  • Escalate issues or delays within the patients pathways after day 5
  • Ensure that Somerset (Cancer Tracking System) remains up to date
  • To act as co-ordinator for the Multidisciplinary Team Meetings in the absence of the site specific MDT Co-ordinator
  • To facilitate collection of data for all cancers and enter this onto an appropriate database
  • To prepare and maintain all required and related documentation pertaining to the efficient and effective functioning of the MDT Meeting
  • To ensure that individual patient treatment/management plans are recorded in the patients notes and that these plans are disseminated to the patient and their General Practitioner within the stipulated time frame
  • To be responsible for the making of appointments, notifying patients and liaising with practices as required by the Cancer Data and Information Manager
  • To ensure that MDT administration meets Trust Operational Policy
  • To highlight capacity issues to Cancer Data and information officer in a timely manner.

Qualifications

  • CLAIT, ECDL or RSA grade 3 in text or processing
