Community Support Worker - District Nursing - Stratford Locality
Stratford Locality - 37.5 Hours
We are looking for a flexible and organised individual to join our team. You will have at least 1 year related experience, preferably in the community and have some knowledge and skills in managing the care of long term conditions and end of life patients.
The successful candidate will be self -motivated, enthusiastic, possess excellent communication skills and be a good team player. You must be able to demonstrate effective interpersonal skills.
You will be working collaboratively with the day service teams and the Community Emergency Response Team to deliver seamless care across the locality, and in particular for our end of life patients.
The post is a community post and the ability to meet the travel requirements of the role is essential.
The community nursing service operates across the hours of 8.30 am to 22:00pm, 7 days a week, including weekends and bank holidays.
You will need to be able to work flexibly and be comfortable with agile working.
We have availability for both or part time hours
To help us achieve our vision of providing the best possible healthcare, South Warwickshire NHS Foundation Trust run Assessment Centre’s to select its future employees. These vary depending on the vacancy but their core purpose is to create an environment in which you, along with other candidates are able to demonstrate key workplace skills.
Assessment is based on a numeracy test, an interview and a group exercise which will be undertaken against the core values and behaviours that our employees pride themselves with.

The Trust is committed to safeguarding and promoting the welfare of individuals and expects all staff to share this commitment. As part of our safe recruitment practice, it is a mandatory requirement for all newly appointed staff requiring a Disclosure and Barring Service (DBS) check to be registered with the DBS Update Service. All eligible staff recruited to the Trust are expected to maintain their annual update subscription whilst employed with the Trust as a condition of employment. The cost of the first year’s subscription to the update service will be reimbursed by the Trust as a payroll refund.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
