Frailty Unit, Band 5 Nurse

South Warwickshire Foundation Trust is looking for enthusiastic nurses, who strive to give outstanding care, to join our frailty ward teams. We are looking for nurses who are highly motivated and want to achieve and deliver the very best in themselves and for their patients.

This is an opportunity to secure an excellent position within a high performing NHS Foundation Trust that provides acute inpatient treatment and has been rated as                    ‘outstanding’ by the CQC.

We have an outstanding preceptorship programme to ensure that you are well supported to get your career off to the best start and to enable you to achieve your potential; including supernumerary time and close work with an experienced member of staff to help you in your new position.

The Team is truly multi-professional, we currently have Consultant Practitioners, Advanced Nurse Practitioners, Physiotherapists, Occupational Therapists, Trainee Nurses Associates, while we all work to our core strengths and abilities, we also help each other to develop new skills and acquire new knowledge that enables us to provide truly holistic patient centered care. The team operates from three bases, each base has an established group of staff to provide continuity to our patients.

Frailty Assessment Unit

The Frailty Unit provides a dedicated specialist team that rapidly assesses patients with symptoms of frailty at the point of entry to the trust. The unit is a new and exciting opportunity for those looking to make a difference to patients with complex needs, long term health conditions and those needing that extra care support with their patient journey.

Frailty – Nicholas & Fairfax Ward

Covering two wards, the frailty unit is an energetic and caring environment with a dedicated multi-disciplinary team. Working collaboratively the wards aim is to improve holistic patient care and work towards clinical excellence at all times. Combined with the assessment ward, the frailty unit aim to improve and individualise care with new pathways and patient focussed working.
