Probationary Period
All new staff to the Trust will be subject to a standard 6 month probationary period, the details of which will duly follow in your contract of employment.

Agile and Flexible Working
All our staff will have a work base. However the Trust operates as an Agile Working Team therefore staff may be working from home on a regular basis as required by the service. However, occasionally travel around Warwickshire and into your base will be required so access to your own transport would be helpful.

Base - Whitnash Lodge/ Agile Homeworking
An exciting opportunity has arisen for an experienced nurse to join a well-established team working within the iSPA Integrated Care Co-ordination Service (ICC).
The teams role is to triage all urgent referrals who require an urgent or same day response from the Urgent Community Response Teams (UCR). ICC also have the ability to sign post to an alternative more appropriate service or provide clinical advice only if required.
Your role will be to safely triage using a sound clinical knowledge base, monitor and triage ambulance referrals through the CAD portal, refer, allocate and support access to out of hospital services, primary and secondary care.
Clinicians will use their sound clinical knowledge and risk assessment skills to clinically triage and clearly identify urgent and non-urgent patient referrals allocating and arranging visits to our patients within the community setting.
ICC are at the forefront of technology with OOHCC and are always looking at interactive resources and systems to provide a smart and efficient service.
Main duties of the job
The post holder will use their sound clinical knowledge and risk assessment skills to prioritise referrals through the clinical triage service as necessary.
The Post holder will effectively coordinate the resources available within Out of Hospital services to maximise access to services or sign post to other appropriate services available.
The post holder will ensure that all contacts with the clinical triage services reach an effective and appropriate outcome for the patient.
Organisational
To provide managerial support to the team including Performance Development Reviews, Clinical Supervision and Recruitment and Selection.
To provide leadership to the clinical team.
To provide knowledge and guidance with respect of clinical health services and to ensure and facilitate an effective interface between primary care services and out of hospital services.
Contribute towards ensuring the AHP & Nursing Strategy is implemented.
Participate in the development, training and mentorship of pre-registration students/medical students and staff as required including induction and preceptorship programmes.
To provide assistance with the resolution of complaints within the clinical speciality, or sphere of responsibility.

Detailed job description and main responsibilities
Main Functions
To signpost, refer and support access to out of hospital services, primary and secondary care.
To clearly identify urgent and non-urgent referrals and allocate patient to the appropriate pathway as appropriate.
To interrogate the Trusts Information systems with the purpose of eliciting patient information to support the decision-making process.
To liaise with relevant Health Care Professionals and Carers (with appropriate consent) to elicit relevant patient information which supports the decision making process.
To work collaboratively with colleagues to provide a high-quality clinical triage service to the Trusts other Health care Professionals.
To build close working relationships with the referral sources by assisting in the development of pathways from the Clinical Triage team to other providers and agencies to ensure that effective sign posting, and pathways are maintained.
Act in accordance with current local Warwickshire Safeguarding Vulnerable Adults and Trust guidelines/policy to protect vulnerable adults.
Act within the NMC Code of Professional Conduct and/or another Professional Group.
Communication
Develop and maintain effective communication networks including within complex situations, with service users, health care professionals and other statutory and voluntary organisations.
Receives highly sensitive information on a daily basis from a variety of referral sources both external and internal.

General
The Trust endorses the principle of whether people smoke or not is a matter of personal choice but where they smoke is a matter of public concern. The policy on smoking and health protects non-smokers and requires that all Trust premises are smoke free.
Physical and Mental Effort

- Registered Nurse (1st Level)/ physiotherapist/Occupational therapist
- Specialist Practitioner Qualification or equivalent (cert/dip/degree) or working towards
- Evidence of relevant post graduate training and CPD
