Probationary Period
All new staff to the Trust will be subject to a standard 6 month probationary period, the details of which will duly follow in your contract of employment.

Job overview
An exciting opportunity has arisen for a dynamic, motivated registered nurse to join the Anti-coagulation Service at SWFT, as an Anti-coagulation Specialist Nurse. The post encompasses all areas of anti-coagulation, and the successful applicant will be caring for patients who are at risk of or being treated for a variety of different thrombosis or emboli.
Management of patients includes the assessment, initiation, management and evaluation of patients on anticoagulation therapy (including Low molecular weight heparin, direct oral anticoagulants – DOACs and warfarin and other vitamin K antagonists). We offer a wide variety of clinics and whilst we do undertake face to face consultations, a significant number are via the telephone. We are also looking to expand inpatient support in the future. The post involves undertaking significant clerical duties. The successful applicant will have good organisation skills and be able to demonstrate the ability to work in methodically and consistently, with the ability to meet deadlines in a timely manner.
Main duties of the job
To provide expert advice and interventions within the anticoagulation service, that maintains and improves standards of care and promotes a seamless service within a multi-professional framework. This will be achieved through clinical practice, management, education, research and audit of professional activities taking into account objectives of the clinical service group, corporate objectives and within the Clinical Governance Framework. The primary role of the post will be to work in the Anticoagulation team using the Raid computerised dosing system. The nurse specialist will act as a clinical expert alongside a fellow Band 6 Nurse and provide a professional source to the multi-professional team, patients and carers within the anticoagulation service. To supervise support staff within the anticoagulant department, keeping the Manager and Clinical Led Consultant informed of any problems.

Detailed job description and main responsibilities

To deliver a patient centred service which involves a consultation and computerised anticoagulation dosing service for a case-load of patients, both in-patients and out-patients. Work closely with the Consultant Haematologist service Lead. To act as a role model and clinical expert for all nursing and medical staff within the Trust and the community extending advice to other healthcare professionals such as General Practitioners. To be responsible for the on-going evaluation of the effectiveness of patient care, including evaluation of own practice and ensure that patient records are maintained in accordance with the Trust policy using paper and electronic records. Ensure patient follow up is rigidly maintained, using computer software, liaising with administration staff and discharging to General Practitioners as necessary. To be competent and confident with the use of Near Patient Testing Devices. Identifying patients who require the domiciliary service, liaising with General Practitioners, phlebotomy staff and district nurses to arrange this. The nurse specialist will liaise closely with the cardiology team; ensuring patients are adequately treated as per NICE guidelines. The nurse specialist will liaise closely with the various medical disciplines for those patients requiring anticoagulant follow-up following venous thrombosis as per the agreed pathway. The nurse specialist liaises with all other specialist fields in the preparation of pre-op and post-op management following policies and procedures. Knowledge skills and experience required: Act as a role model and clinical expert providing specialist, expert clinical knowledge to clinical colleagues, patients and carers / relatives. Undertake clinical supervision of nursing colleagues on an individual or group basis. Participate in the development of a strategic plan for (Specialty) and assist the Manager / General Manager to implement change. Ensure the effective and efficient use of physical and financial resources. Participate in the development of strategies for delivering effective specialist care within a changing environment that improves the quality of care and health outcomes in the specialist area. Monitor health, safety and security of self and others and promote best practice in the service area. To promote and support innovative practice and ensure relevant research findings are incorporated into practice. To contribute to audit and research activities within the speciality and in collaboration with multi-disciplinary colleagues. To participate in the promotion and development of the speciality by publishing and presenting innovations, audit and research findings locally and nationally. To attend regional and national meetings on anticoagulation, such as the RAID Anticoagulation Therapy Management System. Measurable Result areas: Continued professional and personal development. Ensure own compliance and compliance of others with regard to mandatory training and revalidation requirements. Ensure compliance with policies, procedures and clinical guidelines for self and others and participate in the development of the same. Continually monitor standards of care and participate in the improvement of care throughout benchmarking, Nursing Charter Audits, Clinical Audit and Research Evaluate research findings and facilitate evidence based practice in the specialist area. Disseminate research findings and knowledge by pursuing opportunities to present work at national conferences or via publications. Promote patient and public involvement activities in the specialist area leading to service improvement. Promote peoples equality, diversity and rights. Identify clinical issues and incidents within the Trust that reduce the quality of care within the specialist service. Take part in activities to counter these to ensure the delivery of safe and effective care Maintain active status on NMC register. Act always in accordance with NMC code of conduct and guiding documents. Adhere to Trust Policy & Procedure. Maintain a professional portfolio to support revalidation Maintain a high level of awareness of relevant research issues and trends within the specialist area of practice and in nursing generally. Take an active part in the development of nursing in the Trust via participation in professional nursing forum and shared governance initiatives. Take responsibility for personal development and education with regard to PREP and Personal Development Plan objectives. Participate in Clinical Supervision for self and others and utilise reflective practice techniques.
Communications and Working Relationships:
Promote team working and meet with manager and clinical lead to discuss complex cases and participate in shared learning. To provide specialist education to members of the anticoagulation team, which facilitates learning and individual personal development Produce suitable written educational materials for staff, patients and carers, and play an active role in their education. To be involved in the supervision of students as necessary. The coordination of education and training programmes for both nursing and junior medical staff. Demonstrate empathy and compassion to patients and carers throughout their pathway of care and management. Use persuasive and advanced communication skills to deliver sensitive or complex information to reduce barriers in language and understanding.
