Job overview
The post holder will be responsible for assessment and specialist employment advice, support and interventions. They will manage a caseload of people assessing individual barriers into employment, experienced through physical health, mental health and social situation. Using the WorkWell pathway work plan, the post holder will prescribe either Multi-disciplinary in-house support or to triage/signpost and refer. The post holder will facilitate clients being referred to the service quickly so that they do not remain unsupported whilst on sick leave, and supporting them to return to work swiftly (where appropriate), or to follow the destination that is best suited to their circumstances, which enables individuals to achieve positive service experience outcomes.

Black Country Healthcare NHS Foundation Trust is recruiting 2 WTE WorkWell Work and Health coaches to be based in our WorkWell service , which supports individuals who maybe experiencing multiple and complex barriers preventing them from accessing employment opportunities or retaining their employment.
The roles will support individuals across the the Black Country , and will form a key part of individuals’ recovery and health care plans. The successful candidates will support a caseload of individuals who have a health condition and have experienced the above barriers, who are unemployed, or employed but require retention support. This is an exciting opportunity to be a part of a new Black Country wide initiative team.
