Job overview
An exciting opportunity has arisen for a Band 6 Community Mental Health Practitioner to join the West Sussex Specialist Older Adults Service Triage Team based at Drayton House/work from home.
We are a friendly and welcoming multi-disciplinary community team, who triage the referrals for all of the older adults services across West Sussex.
Although we are remote/working from home we do meet once a month in person as well as daily meetings over teams. Ideally you will be located in the West Sussex locality.

Main duties of the job
· To screen and determine the clinical pathway for all patients referred to the service.
· To work in partnership with the service users and their families/carers.
· To demonstrate an understanding of clinical interventions relevant to our care group including education, signposting, pathway planning.
T To have experience of and specialist knowledge of interventions for our service users and referrers and provide specialist nursing advice.
· To receive management supervision from the Clinical Nurse Specialist or equivalent.
· To demonstrate excellent communication skills.
· The aim of this role is to ensure that there is a consistent approach to triaging and processing of referrals that are received.

Detailed job description and main responsibilities
This community nurse role will provide a triage and information service to patients referred to SOAMHS in West Sussex. The post holder will be a part of the team that will screen and determine the clinical pathway for all patients referred to the service. The majority of referrals will be made via the electronic referral system.
The role of Community Nurse is to work in partnership with service users in the designated care group with complex health presentations, They will demonstrate an understanding of clinical interventions relevant the care group to include education, signposting, pathway planning, working closely with carers, families and partner agencies to facilitate this. The post holder will work at all times to promote the safety and the well being of service users and their families/carers..
You will provide clinical supervision to staff and students.
Essential
- Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role
- Completion of Mentorship Course/ENB equivalent
- Evidence of continuous professional development
Knowledge/Experience - Essential
- Significant post qualifying experience in the relevant care group
- Experience of working alongside people with complex health problems in the designated care group
- Experience of supervising staff
- Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
- Experience of working consultatively with professionals
- Experience of conducting clinical assessments including risk assessments
- Experience of partnership working within and across various statutory and non statutory teams and agencies
- Experience of working within the CPA process
- Understanding of clinical governance
- Previous experience of working in the community
