Job overview

The East Sussex Rehabilitation Pathway offers inpatient and community based rehabilitation. 

This particular post is for a Band 6 Mental Health Practitioner in the community Hastings Hub. We are a multi-disciplinary team who work creatively and dynamically in our approach to those under our care. We are a supportive team and work closely to maintain a joined up approach. 

Within the community we offer both long and short term interventions - 

Short term community interventions – we are able to offer up to 12 weeks of interventions with the aim of providing practical support to maintain independent community living and to access other services / resources within the community. The East Sussex Rehabilitation Team will work alongside the lead practitioner within the referring service, with medical and lead practitioner responsibility remaining with the referrer. 

Long term community interventions - support service users for up to 3 years. The East Sussex Rehabilitation Team will take on medical and CPA responsibility for the service user.

We are looking for an individual who is compassionate,  creative, innovative individual who is able to work well within a MDT.  

Main duties of the job

The post is suitable for a registered Community Mental Health Nurse, Occupational Therapist or Social Worker. The role of postholder is to work in partnership with service users in the designated care group with complex health presentations, developing their care plans and recovery plans to enable them to lead fulfilling lives.  They will demonstrate an understanding of clinical interventions relevant the care group to include education, care plans and discharge planning, working closely with carers, families and partner agencies to facilitate this.  The post holder will work at all times to promote the safety and the well being of service users and their families/carers.  The post holder will assess, plan and implement care, and provide specialist mental health advice and promote recovery.  They will provide clinical supervision to staff and students and act as a mentor for student nurses.

Detailed job description and main responsibilities

The post holder will be an autonomous practitioner who will carry continuing responsibility for a defined caseload of service users by undertaking the following duties:

To undertake assessments of individuals and their families in the designated care group with complex health presentations, including those service users presenting with higher levels of risk. To develop alongside the service user their care plan.

To offer specialist clinical assessment and advice where required.

To assess and manage on going risks as identified during the assessment, ensuring that this is done in collaboration with the service user and carers.

To regularly review risk factors and make changes to the management of them as necessary.

To participate in the planning, development and evaluation of profession specific and local multi-professional services.

To ensure that clinical practice is evidence based and consistent with relevant NICE guidelines and Trust policies and procedures.

To provide supervision of junior staff and trainees where appropriate.

To act within the relevant professional code of conduct at all times

Person specification

Experience

Essential criteria
  • Registered Mental health Nurse Occupational Therapist or Social worker with current registration
  • Completion of Mentorship Course
  • Evidence of continuous professional development

Knowledge/Experience

Essential criteria
  • Significant post qualifying experience in the relevant care group
  • Experience of working alongside people with complex health problems in the designated care group
  • Experience of supervising staff
  • Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
  • Experience of working consultatively with professionals
  • Experience of conducting clinical assessments including risk assessments
  • Experience of partnership working within and across various statutory and non statutory teams and agencies
  • Experience of working within the CPA process
  • Understanding of Clinical Governance
  • Previous experience of working in the community
