Job overview

An exciting opportunity is available as a Band 4  Senior Physical Health Support Worker join the Wealden Neighbourhood Mental Health Team (NMHT).    We are looking for an individual who has a genuine interest in physical health working with service users within the mental health service.  The successful candidate will be able to work autonomously along as working as part of a well-established team within the health and well-being clinics. 

 

You will be responsible in coordinating and running physical health clinics, health assessments, and relevant physical health interventions to support service users with mental health conditions to improve their physical health.   

 

Extensive knowledge of running a clinic, alongside physical health monitoring such as ECG's, Venepuncture and escalation of physical health concerns to the appropriate teams is essential.

 

We welcome applications from experience Physical Support Workers and Registered Nurse Associates.

Main duties of the job

Main duties of the job

  • ·       Organise physical health clinics, liaising with service users to confirm appointment slots and organise patient transport.
  • ·       Ensuring physical health stats (weight, height, BP, Temp, BMI), Blood tests (lipids, HbA1c, FBC, Glucose) and ECGs are completed within a set time frame.
  • ·       To support the implementation of local clozapine clinics including the use of a Yumni Machine.
  • ·       To support service users to think more about their physical health needs and identify their own physical health goals.
  • ·       Provide advice on weight loss, exercise and raise referrals to external agencies such as CGL (previously STAR), OneYou etc.
  • ·       Ensuring patient information is entered on SystmOne promptly and accurately.
  • ·       Ensuring clinic resources are audited and well stocked.
  • ·       Working alongside team practitioners and access facilitators based in GPs to support service users with complex health needs.

This post is part of a multi-disciplinary team providing a service for people with severe and unstable mental health needs within the High Weald and Lewes and Havens locality.  The Service includes Psychiatrists, Nurses, Occupational Therapists, Psychologists, Support Workers and General Mental Health Practitioners.
