Job overview
We have a great opportunity for full-time band 7 Lead Nurse Mental Health Practitioner to lead Dementia Care Home Support Team . This service operates a Monday - Friday 9-5. The successful candidate will join a small friendly, highly-skilled and enthusiastic multi professional team. We will take a keen interest in helping you to develop the skills important to this role and the post-holder will be encouraged to develop special interests and expertise in line with service and team objectives.
You will receive regular managerial and clinical supervision. This is a challenging but rewarding work as we receive positive feedback from our service users and carers. Sussex Partnership NHS Foundation Trust has recently achieved Outstanding from CQC for the care it provides.
The post holder will be expected to travel across the geographical areas of Costal areas covered by the team, so the ability to travel to remote areas is essential.
Main duties of the job
The role of this post is to actively engage those requiring an increased level of support with the aim of keeping people within Care homes and maintaining their wellbeing and independence as far as possible. Avoiding admission to general hospitals and support discharge from general and mental health hospitals. We are looking for an individual who is dynamic, compassionate, good communicator who are able to demonstrate a holistic and person-centred approach to their work.
You will also need to be able to work cohesively with other services both within our Trust, and with other partner agencies. We follow a multi-disciplinary approach and within that context you will manage a caseload of older adults as a team.

Detailed job description and main responsibilities
The role of Mental Health Practitioner is to work in partnership Older people in the designated care group with complex health presentations, developing their care plans and recovery plans
to enable them to lead fulfilling lives. They will demonstrate an understanding of clinical interventions relevant the care group to include education, care plans and discharge planning, working closely with carers, families and partner agencies to facilitate this. The post holder will
work at all times to promote the safety and the well being of older adults and their families/carers. The post holder will assess, plan and implement care, and provide specialist advice and interventions. They will provide clinical supervision to staff and students and act as
a mentor for students.

Qualifications
Essential criteria
- Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role
- Completion of Mentorship Course/ENB equivalent
- Evidence of continuous professional development
Knowledge / Experience
Essential criteria
- Significant post qualifying experience in the relevant care group
- Dementia Experience
- Experience of working alongside people with complex health problems in the designated care group
- Experience of managing a caseload
- Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems including specialist assessments
- Experience of working consultatively with professionals
- Experience of conducting clinical assessments including risk assessments
- Experience of partnership working within and across various statutory and non statutory teams and agencies
- Experience of working within the CPA process
- Understanding of clinical governance
