We are currently welcoming applications for a Community Staff Nurse to join our dynamic and supportive Community Nursing Team, serving the South and City areas of Bradford.

As part of our Planned Care Service, you’ll deliver high-quality, person-centered care to individuals in their home environment—supporting a wide range of needs including long-term conditions, complex care, and palliative support. You’ll work on a rotational basis across a seven-day service, with shift patterns between 8am and 9pm, supported by agile technology and a multidisciplinary team of specialists across the Trust

Main duties of the job

To work as part of the district nursing team to ensure high quality health care is delivered to patients in their own home and/ or clinic setting. To work in collaboration with the wider Primary Health Care team, whilst participating in the assessment of health needs and delivery of care to the practice population. The service is delivered over a 7 day rota and postholders are expected to work flexibly in line with agreed operating hours to cover the service
