VIRTUAL WARD Frail Elderly- Community but not as you know it!
This is an exciting opportunity to join an Award Winning Multidisciplinary Community team.
Varying shift patterns available to cover days and nights. Family friendly and part time hours considered, please ring if you have specific requirements which you would like to discuss.
The Community Elderly Virtual Ward Team is made up of Nurses, Occupational Therapists, Physiotherapists, Rehabilitation Support Workers and Healthcare Assistants along with a Consultant.
We provide care in a patients home therefore a full UK driving license and access to a car is essential. Overseas driving licence will not be accepted.
The post holder will deliver seamless care across organisational boundaries reviewing patients in a community setting in order to improve the ability for patients to self-care; to enable patients to recover at home following illness/ preventing the need for patients to be admitted to hospital.
Main duties of the job
The team supports patients with the challenges of rehabilitation and work to achieve the optimum abilities of the patient under the direction of a therapist/senior nurse.
Staff newly appointed into the service will be provided with a competence package and they will be supported and developed in this role to provide a first class service to our patients.

Detailed job description and main responsibilities
To deliver high quality time limited programmes of nursing and rehabilitative care to individuals within intermediate care services. Provide support to people with older age, long term conditions and/or rehabilitation needs. To support the development of the team and provide a consistently high standard of care to patients and their carers.

- Experience of working as part of a team
- Understanding of the rehabilitation philosophy of care
- Experience of working with older people or people with disabilities
Skills
Essential criteria
- Demonstrate the importance of enablement
- Willingness to undergo training and work towards professional performance objectives
Desirable criteria
- Demonstrate an understanding of the role of health, social and therapy services
Qualifications
Essential criteria
- Diploma level 3 in Health and Social Care
- Grade A-C GCSE Maths and English
Desirable criteria
- Evidence based therapy competencies ( Calderdale national framework)
Knowledge
Essential criteria
- Demonstrates level of self-awareness- strengths, weaknesses, personal qualities
- Able to demonstrate IT skills and knowledge
