Our team delivers anticipatory care to adults with multiple long-term conditions in a defined area, through referrals and case finding. We aim to reduce health inequalities, prevent unnecessary hospital admissions, and help patients reach their full potential through a multidisciplinary, individualised approach.

You will work closely with Physiotherapists, Occupational Therapists, and other therapy assistants, providing effective therapy treatments in patients’ homes.

The successful applicant will have experience working with Occupational Therapy and Physiotherapy, with an interest in delivering therapy within the community and supporting patients in their own homes.

As part of a multidisciplinary team with shared roles, you will help reduce admission risks and maximise patients’ potential. You will assess, evaluate, and treat complex patients while collaborating with health, social, and voluntary sector partners to deliver evidence-based care.

You will be supported by the MDT to develop your clinical skills through shadowing, competency-based training, and regular supervision. This will build your confidence and clinical reasoning, enabling you to work independently and manage your own caseload.

Prif ddyletswyddau'r swydd

To deliver a high standard of health care supporting both Occupational therapists and Physiotherapists, together with the wider MDT, to assess, plan and implement a range of therapeutic interventions to improve the physical, cognitive and functional ability, mood and well-being of patients. 

To undertake assessments and treatments for both physiotherapy and occupational therapy independently.    

There will be additional generic duties as directed by the wider MDT.

To facilitate safe and timely interventions.

To manage and prioritise own caseload delegated by qualified therapists / MDT evaluating and adapting interventions.  

To contribute to team and service development.

To contribute to training and support to other team members and students.

The post holder will be required to work flexibly across the Keighley and Bradford bases as the service required.

  • Therapy Assistant experience in an NHS Setting/ equivalent experience.
  • Evidence to demonstrate own learning and to support experience

  • Experience of carrying out assessments and/or therapeutic interventions with people in an acute hospital setting
  • Experience of assisting in the therapy provision for patients with complex and /or progressive conditions.
  • Experience of lone working in the community setting
  • Experience of practical training/supervision/appraisal and development of Band 2 therapy assistants
  • Experience of data collection for audit as directed by qualified therapists

Skills

Qualifications

  • Degree Apprenticeship Healthcare Assistant Practitioner - Level 5 Foundation degree Successful candidates who do not have a foundation degree or equivalent in a relevant area will be required to work towards this.

  • GCSE Maths and English or Maths and English – level 2 functional skills
  • NVQ Level 3 study or evidence of level 3 study or evidence of extensive clinical experience in a health and social care
  • Courses with relevance to the job role.
