Job overview
The post holder will be responsible for their designated workload within their speciality area with access to the support, advice and management from a senior clinician whilst not directly supervised. In doing so, they will assess, plan, deliver and evaluate patient centred programmes of care ensuring that they practice within their sphere of competence and knowledge. The post holder will work to professional protocols and regulatory body codes, standards and guidance always ensuring that their practice is grounded in the best available evidence based theoretical and practical knowledge. The post holder will provide medicines management within a community setting.

Main duties of the job
Working as part of a multi-professional disciplinary team and providing a holistic oversight of medicines management processes undertaken in the Proactive Care team.
Supporting, teaching, and providing advice and support regarding best practice guidance in reference to medicines optimisation and medicines management. To optimise patient care and ensure that where possible that patient independence is preserved and maintained.

Qualifications
Essential criteria
- NVQ Level 3, BTEC, or similar nationally recognised qualification in Pharmaceutical Sciences
- Registered with the General Pharmaceutical Council (GPhC)
Desirable criteria
- Accreditation in Medicines Management or equivalent experience
- Possession of Accredited Checking technician qualification
Experience
Essential criteria
- Appropriate demonstrable post-qualification Pharmacy work experience, covering the main skills areas, and including the relevant specialty
- Patient consultation skills
- Experience of working to standard operating procedures
- Experience of medicines optimisation
Desirable criteria
- Experience of working in a similar role
Knowledge
Essential criteria
- Knowledge of Medicine management
- Evidence of continued professional development in line with GPhC registration.
- Understanding of health services and medicines/ pharmacy related issues in primary and secondary care
- To have knowledge/ experience of quality improvement, service development and successfully managing service delivery/ changes
- Knowledge and understanding of pharmaceutical technical procedures specific to the area of work
Desirable criteria
- Accurately record all issues, updating the patient records on SystmOne
- Knowledge of electronic prescribing.
