Community services keep people well, manage acute illness and long-term conditions as well as helping people to live independently.   In order to deliver the Long Term Plan, we need to work across integrated care systems to address ongoing challenges to community provision, amplified by the recent COVID-19 pandemic.

The post holder will lead a forming community services team to design, test and implement critical changes to strengthen community services across the South East Region.

We are looking for a dynamic health service leader who is equipped with the skills, knowledge and pragmatism to oversee development of the region’s Community Programme.

Please note that the reason for the fixed term of your contract is short term project due to funding.
