Job overview
We are looking to appoint a Band 5 Staff Nurse who is enthusiastic and motivated to join our established integrated intermediate care team, based at Castleberg Hospital in Giggleswick.
The Intermediate Care Teams have undergone recent developments and have expanded to include Castleberg Hospital (10 beds). The 2016 CQC report described the Intermediate Care Teams as an outstanding example of community working.
The service at Castleberg provides bed-based intermediate care to support people who are in an acute but stable condition. The service also provides palliative care to people during their last days of life if they cannot be cared for safely in their own home.
The Intermediate Care Team’s includes Physiotherapists, Occupational therapists, Advanced Clinical Practitioners, Nurses and Support Workers. It is important to be able to work within a multidisciplinary team as well as independently. We utilise the Calderdale competency framework for skill sharing
Applicants must be prepared to work shifts covering 24/7 and have access to own transport, car driver and access to vehicle essential (Car leasing schemes available).
Main duties of the job
Nursing and rehabilitation for inpatients at Castleberg hospital
- You will provide high quality nursing care to a range of people with varying health care needs
- We push the boundaries of what care is achievable in a community hospital
- work closely with the MDT

Qualifications
Essential criteria
- Registered Nurse (Adult)
- Current NMC Registration
- Post qualification clinical experience
Desirable criteria
- Teaching & Assessment Qualification
Experience/Knowledge
Essential criteria
- Able to assess patients with comple health care needs and develop care plans to meet needs.
- Experience of risk management and governance.
- Demonstrate an understanding of the role of intermediate care services in the community
- Knowledge of long term conditions
- Understanding of the NHS Constitution and its implications
Desirable criteria
- Palliative care experience
- Phlebotomy experience
- Post registration experience
- Use of SystemOne and EPMA (Electronic Prescribing Medicines Administration)
- Community nursing experience
