Job overview
The post holder is responsible for overseeing the management of the Clinical Informatics Audit team and the health records processes and team. In addition they are responsible for:

Providing leadership, through their conduct and actions as per trust values acting as an ambassador for the clinical quality strategy. Provide direct line management, supervise, coach and develop staff in the CIA and records teams.

Oversee the quality and production of all clinical audits, ensuring audit methodology and risk management approaches are followed by the CIA team.

Develop and follow clinically credible architecture for future digital reporting relating to clinical audit and clinical data and implement a monitoring process that evaluates improvements and or change to practice.

Produce detailed and relevant reports for presentation to clinical governance group, medicine optimisation group and other forums to provide trust assurance on the quality of care

Produce audit papers for local and national publication through HQIP
-
Undertake the monthly review of the NICE, NCPOD, HQIP and other national guidance for distillation and review by subject matter experts, prepare quarterly reports for CGG on actions and monitoring of impacts.

Is clinically credible and able to liaise with senior clinicians, facilitating review and development of audits and standards as well as support processes for system evaluation following improvement interventions

Provide a resource to operational and other teams to support clinical audit

Provide a link for acute trusts to share audit information and learning

Undertake investigations both clinical and non-clinical as necessary

Responsible for ensuring that IG practices are adhered to in clinical audit, data share, and records management.

Main duties of the job
Core Responsibilities
Communication and Relationship Responsibilities
The post holder will ensure clear communications between the CIA and health records staff fostering the trusts values through one team leadership and management. They will ensure the department’s Staff are informed of objectives and key milestones.

Work with HR and recruitment business partners to support staff and follow organisational policy and procedures. Support with investigations and fact find as required.

A key relationship is with the Head of Clinical Effectiveness and Governance the post holder will provide assurance relating to the quality of clinical audits produced; clinical aspects of reports are sound and that the audit programme is delivered.

Form partnerships with local Clinical Governance leads, and Clinical Development Mangers advising on improvements following audit results for trust and local levels

Work with systems and development teams to support the development of the information portal to maintain the un-finalised ePR to minimum levels.

Write and present trust level reports for CQDF and CGG

Provide audit advice for the development of the ePR and PCR ensuring this innovation continues to be current and relevant.

Ensure the production of records guides to support the heath records team in undertaking their roles

Link with IT in the development of the Onbase and ‘Readsoft’ programmes

Oversee external requests for clinical information with acute trusts building relationships that enhances information sharing and learning

Provide local operational management teams and staff with structured reports, outlining local clinical performance, (ACQI and local audits).

Provide challenge to decisions and escalate through reports if patient safety is not taking primacy as re-audits highlight such practice.

The post holder will have a key role in working with other clinical directorate team members to support the delivery of the clinical strategy. The production of quality information that outlines the care delivered by the trust

Represent the directorate and line manager at local and regional meetings where clinical quality governance and learning from incidents are related.

Liaise with communications teams to ensure audits are published and accessible to staff and key learning highlighted

Locally manage the budget; sign-off of expenses, timesheets and authorise overtime .

Detailed job description and main responsibilities
Analytical and Judgemental Responsibilities
The post holder will undertake review of clinical audit reports highlighting relevance undertaking the evaluation and quality review of all clinical audits prior to publication.

They are responsible for the review and update of health record completion guides and clinical audit and record keeping policies.

They will evaluate the impact of ePR and review the access to the portal, review of all unfinalised records and creating reports
-
They will identify incidence of patient safety or other risks; escalate these to clinical and operational teams to ensure patients remain safe in our care.

Undertake a review of all clinical audits and issues from clinical cases to provide recommendations for the trust to adopt for improvement
-
Review of SOP and system processes to ensure they are current and fit for purpose
-
Provide guidance on the use of data presentation and relevance in use of graphs / infographics
Planning and Organisational Responsibilities
- Plan own workload and support others to deliver objectives.
-
Undertake meaningful staff PDR and review ensuring the teams are well developed and fit for the role.

Ensure there is adequate staff capacity to undertake the audit programme or other quality standards, escalating to line manager when there are risks or other challenges.

Develop a planner for and delivery of reports and updates to ensure these are timely, instate monitoring of actions for improvement as requested

Oversee the adherence to the clinical audit programme monitoring monthly and adapting to organisational requirements

Direct the processing of clinical records both current and archived ensuring the system is functional

Organise meetings with internal and external partners including: team meetings and CQDF and HQIP (YEARN)

Ensure relevant reports are available for trust meetings
Policy and Service Development Responsibilities
- Work with the HofCE to develop policies related to audit and health records
-
Plan and implement service developments for the local improvement of ACQI/medicines and other clinical audits. This will include outlining service developments for training and education or changes to local practice where outlined within findings of an investigation

Responsible of highlighting good practice for Ambulance Systems Indicators (AmbSYS) and Clinical Outcomes (AmbCO) and sharing learning with others

Ensure you provide the opportunity for the teams to develop by undertaking quality appraisals.
Financial, Physical and Information Resource Responsibilities
- To ensure all NHS resources are utilised to best effect including use of staff time e.g. use of Ms Teams for meetings limiting travel

Research and Audit Responsibilities

Support the development of systems to capture and audit information that highlights clinical performance issues

Demonstrate ability and expertise in undertaking clinical investigations through clinically relevant and current records review, clear questions, supported staff, well-informed reports

To develop and professionally present information at a number of forums including coroners; family briefings and mortality and LIR groups.

Qualifications, Skills, Experience
Essential criteria
- • Expert in applying and sharing the clinical audit methodology across an organisation
- • Has experience of managing staff and a department budget
- • Self-motivated, able to prioritise own work and tasks to meet the needs of the organisation
- • Competent in report writing; analysis and presenting recommendations
- • Experienced in leading, supervising and coaching members of staff
- Qualification in or specialist knowledge of clinical data analytics and data presentation
- • Professional Audit/Research qualification - Masters or Equivalent Knowledge
Desirable criteria
- • Has worked in or with clinical records processing including working with data storing and retrieval systems
