Job overview

This is an exciting secondment opportunity to work within the CHFT multi-disciplinary specialist respiratory team.  The post holder will take the lead on attaining accreditation for the pulmonary rehabilitation service working alongside the existing clinical lead physiotherapist. The team delivers a community pulmonary rehabilitation service that supports and promotes optimal self-care. This service may be delivered to a cohort in a class setting, individually tailored home based rehabilitation as part of a supported discharge pathway, or virtually using digital and remote platforms.

The Pulmonary Rehabilitation Service Accreditation Scheme (PRSAS) is run by the royal college of Physicians. The aim of the programme is to improve the quality of pulmonary rehabilitation across the UK. The post is a 12 months Secondment/fixed term temporary position. The postholder will work alongside existing clinical staff to review current provision of the service and to then manage the process of meeting the established standards needed to become an accredited service. There will be a small element of clinical caseload, running groups and supervision of less senior members of the team in the absence of the existing band 7 clinical lead physiotherapist.

Experience in project management skills will be useful for this position.  Previous experience of working within the respiratory field in particular pulmonary rehabilitation is essential.  Knowledge of the PRSAS is an advantage.

 

Main duties of the job

To Act as a change agent and innovator, planning, instigating, and evaluating change in line with the pulmonary rehabilitation accreditation standards. To assist the already established pulmonary rehabilitation team implement standards to help achieve accreditation.   This will be achieved by developing and maintaining collaborative relationships, partnerships, and networks within the local region

The postholder will contribute to measuring the effectiveness and productivity of pulmonary rehabilitations services provided through the collection and correlation of data for the team annual report and ongoing national audit 

The post holder will support the pulmonary rehabilitation clinical lead in the daily operational management of pulmonary rehabilitation and manage autonomously a small clinical workload of patients with chronic respiratory disease at a highly specialist level, assessing and treating patients and maintaining clinical records.

Detailed job description and main responsibilities

·      Act as a change agent and innovator, planning, instigating, and evaluating change in line with the pulmonary rehabilitation accreditation standards

·  To promote the provision of pulmonary rehabilitation across Calderdale and Huddersfield NHS Foundation Trust and GP practices within Calderdale

·     To develop and maintain collaborative relationships, partnerships, and networks to influence and improve health outcomes and health delivery systems.

·   To ensure service developments for Chronic respiratory disease improve practice and health outcomes that are underpinned by local and national guidance.

·   To contribute to measuring the effectiveness and productivity of pulmonary rehabilitations services provided through the collection and correlation of data for the team annual report and making recommendations

·   To assist other team members in implementing evidence based clinical practice within all areas of the pulmonary rehabilitation programme

·  To work collaboratively with members of the wider community respiratory team to ensure share knowledge and embed good practice and educate around the pulmonary rehabilitation programme.

·       To develop and maintain collaborative relationships, partnerships, and networks to influence and improve the delivery of pulmonary rehabilitation

·       To support the pulmonary rehabilitation clinical lead in the daily operational management of pulmonary rehabilitation.

 

·       Manage autonomously a clinical workload of patients with chronic respiratory disease at a highly specialist level, assessing and treating patients and maintaining clinical records.

·       To undertake the comprehensive assessment of patients, including those with a complex and multiple presentations, using investigative and analytical skills then formulate individualised management and treatment plans using clinical reasoning utilising a wide range of treatment skills and option

·      Formulate and deliver an appropriate individual, evidence based physiotherapy/pulmonary rehabilitation treatment programme based on highly specialised knowledge. This necessitates high levels of autonomy and the analysis of information, interpreting results to inform assessment and treatment planning, selecting an appropriate treatment/management programme, often from a range of options, and evaluating the effectiveness of treatment interventions.

  • Develop effective links with other community and specialist respiratory services including General Practitioners, Practice Nurses and Respiratory Physicians, mental health services, and Community Pharmacists; To provide specialist advice where needed and refer to other agencies as required.

·        Take part in the education and training of Physiotherapists and qualified and non-qualified healthcare / social services / education staff including medical staff.

·     To liaise with the respiratory nurses, to ensure avoidance of admission/readmission and to ensure a swift return to an improved level of independence.

·                             Professional Duties

·             Work autonomously within Trust policies and professional practice, maintaining accurate and comprehensive up to date records.

·               ·                Work in accordance with all Trust policies and procedures.

·                Attend induction and regular update training on risk management policies and procedures.

·                Identify through risk assessment, any risks that exist within their department or in the community during the delivery of the service.

·                Report clinical incidents/near misses or any unexpected event or incident that occur.  To work with the clinical lead, assisting them in the management of complaints and incidents.

·                Maintain confidentiality of all patient identifiable information in line with Trust policies relating to the Data Protection Act.

 

Managerial Duties

·        To lead the process on uploading evidence onto the PRSAS website ensuring it meets the standards.

·   Provide advice / information to the Clinical Lead to help in the planning and development of the service. 

·          Assist the clinical lead in achieving the service objectives.

·       Assist the Clinical Lead in the monitoring of professional, clinical and departmental standards and assist implementation of change as appropriate.

QUALIFICATIONS / TRAINING

Essential criteria
  • Physiotherapy Degree
  • Registered with HCPC
Desirable criteria
  • Member of the Professional body
  • Member of a respiratory network/ACPRC/BTS
  • Previous project management work
  • MSc or equivalent level post graduate study within respiratory field and/or project management

KNOWLEDGE, EXPERIENCE & EXPERTISE

Essential criteria
  • Excellent clinical reasoning skills and the ability to maintain accurate records
  • Highly Specialist level of knowledge and clinical reasoning skills to support expert clinical practice in Pulmonary rehabilitation
  • Significant previous experience in Pulmonary rehabilitation including Planning and delivering programs, and delivering group education sessions
  • Previous involvement in the Pulmonary rehabilitation service accreditation scheme (PRSAS) working towards accreditation within a clinical setting or being involved in the process as an external participant or other healthcare agency
  • Significant experience in contributing to service improvements working with or as the lead in audit or service change/development project
  • The ability to lead assessment and treatment sessions and create detailed and comprehensive treatment plans for complex patients with chronic respiratory disease
  • To have an excellent understanding of the pathophysiology of chronic lung disease applying this to a detailed patient assessment and the further prescription of an exercise plan
  • Knowledge of relevant national and local legislation relevant to pulmonary rehabilitation
  • Excellent IT skills with a variety of IT programmes
Desirable criteria
  • Previous practical experience of performing oxygen assessment and prescription for ambulatory and/or long term therapy
  • Previous experience in leading service improvement/ project management
  • Ability to chair meetings and facilitate workshops effectively
