Job overview
The post-holder will provide high-quality physiotherapy assessment and treatment to patients presenting acutely within the Emergency Department (ED), Short Stay Emergency Unit (SSEU), and Acute Medical Unit (AMU), working within a Same Day Emergency Care (SDEC) model. Working as part of a multidisciplinary frailty team, the role focuses on rapid, holistic assessment, personalised care planning, and early decision-making to support safe discharge, admission avoidance, and improved patient flow.
The role also involves working across professional boundaries to support a wide range of patients with complex needs, leading elements of service delivery, and supporting junior staff within the frailty therapy team. This role offers the opportunity to work flexibly across a seven-day rota, including approximately one weekend shift per month.
Main duties of the job
This role involves providing holistic assessment and treatment for patients with long-term conditions and frailty within an urgent care setting. The post-holder must work flexibly across traditional professional boundaries to ensure continuity of care for patients presenting with a range of conditions, including cardiovascular, respiratory, musculoskeletal, orthopaedic, and neurological issues.

Criteria
Essential criteria
- Degree in Physiotherapy or equivalent
- HCPC registration
- Specialist knowledge in the field of frailty syndromes and interventions for older people.
- Broad experience of working at Band 5 level or above in the NHS (preferably 2+ years).
- Experience at Band 5 or student level in a broad rotational post including cardiorespiratory, orthopaedics, neurology, MSK, and elderly therapy within the NHS
- Experience in the effective supervision and management of junior staff
- Evidence of and desire to supervise students.
- Evidence of CPD, detailing clinical experience, relevant to the post.
Desirable criteria
- Post graduate study/ training in older persons and frailty.
- Be a member of frailty special interest group e.g., British Geriatric Society (BGS) or Society for Acute Medicine (SAM)
- Previous B6 experience in NHS
- Experience in NHS acute/community settings
- Experience of ‘Front door’ working- Emergency department or Acute medical unit
- Experience of complex moving and handling assessments and treatment plans
