Are you ready to be part of a new initiative at Salisbury Hospital?
The Trust is investing in our Early Supportive Discharge team to enable them to support patients home as soon as medically fit with additional care provided by the Council. The service works to a 'discharge to assess' model, in partnership with Wiltshire Council's re-ablement team to provide therapy in the patients own home and ensure patients are given the support they need.
The team is based at the hospital, facilitating discharge plans and liaising with other services before taking patients home. ESD team complete assessments in patients own homes to ensure they have the adequate care and therapy they need to support their return to independence. You will be expected to effectively communicate and coordinate with the team, whilst also using appropriate communication approaches to clinically assess and review patients. You may also be involved with providing care and practicing exercise plans in patients home.
If you are able to use your own initiative, be flexible in your approach, and are motivated to be part of a developing service, with patient centred rehabilitation at its core, then this will be the right post for you. The ESD team is a 7 day service from 08:00-18:00 therefore flexibility in working pattern is required. A standard working week is 08:00-16.00. Having a full driving license and access to a car is essential.
Main duties of the job
You will be expected to treat and support patients who have been identified for the Early Supported Discharge (ESD) Team either as an inpatient or in the community. You will be expected to work autonomously although with the support of a band 6 and wider team, setting treatment goals, monitoring patient progress, encouraging independence with activities of daily living and adapt treatment and rehab plans as necessary. You will also be required to support the daily running of the ESD team and contribute to driving motivation and a positive ethos throughout the team.
