Job overview

The Peri-Operative Care Coordinator is a crucial role in the peri-operative service, responsible for a wide range of clinical, organisational, and administrative duties. They will serve as the primary contact for patients, ensuring their well-being and safety throughout the surgical pathway. The coordinator works closely with the Clinical, Admissions and POAU teams to support the evolving peri-operative service, addressing any barriers and escalating issues that require clinical decision-making. Their day-to-day tasks involve liaising with patients incl. signposting to services, organising clinics, managing administrative responsibilities, and maintaining effective communication with various departments and external agencies. Patient care is paramount in this role, and the coordinator works independently, using their planning, prioritisation, and adjustment skills to provide a seamless and efficient service.

Main duties of the job

  • Coordinators work under supervision of pre-operative assessment nurses.
  • Demonstrate communication skills verbally and in written form in relation to the project delivery.
  • Coordinate detailed referrals from health and social care professionals, communicate and answer queries from all disciplines.
  • Timely surveillance of peri-op patients under optimisation prior to surgery, flagging to the clinical team when further action needs to be taken according to peri-op SOPs
  • Ability to look up test results on the within both primary and secondary care EPR systems and flag to the clinical teams as per peri-op SOPs
  • Ensure the patients receive good pre-operative information in terms of signposting, referrals into appropriate Peri-Operative services, timeframes and health promotion.
  • Timely surveillance of peri-op patients under optimisation prior to surgery, flagging to the clinical team when further action needs to be taken according to peri-op SOPs
  • Carry out timely and accurate data collection and information analysis and reporting in an agreed manner and facilitate testing.
  • Triage patients using tools and guidelines to ensure the patients on the elective surgical waiting list are booked in a timely manner for a surgical procedure.

Essential criteria
  • Care Certificate
  • Level 3 NVQ or equivalent in a Health Care related subject
  • Recent experience of working in a primary or secondary healthcare setting.
  • Ability to work as a member of a team and ability to work without supervision and use own initiative.
  • To communicate effectively with patients both in face-to-face situations, and via the telephone, managing the barriers to communication.
  • Good IT skills
Desirable criteria
  • Involvement in service improvement projects relevant to clinical area and role.
  • Excellent IT skills - Use of Microsoft applications such as – Word, Excel and PowerPoint
