An exciting opportunity has arisen to join the integrated diabetes nursing team within Swindon. This post will be based within the community diabetes team with a focus on providing education to people with Type 2 diabetes. There will also be a requirement to support clinical activity within the team and to provide support and education to health care professionals.
Main duties of the job
The Diabetes Nurse Educator will be a registered professional working within the integrated diabetes nursing team with a specific focus on providing structured education sessions for people living with type 2 diabetes. The role will also include supporting clinical activity within the team and providing support and education to health care professionals in addition to people living with diabetes and their family / carers.
The Diabetes Nurse Educator will be committed to promoting improved quality of life, for the individual living with diabetes and their family.

Detailed job description and main responsibilities
1. Formulate and fulfil own personal development plan.
2. Take responsibility for education sessions for patients with diabetes.
3. Implement systems to enhance quality assurance in diabetes care.
4. Contribute to the development and accreditation process of our trust structured education programme “ On track with Diabetes”
5. Assist in the development of policies and clinical guidelines in support of the specialist nursing service.

Qualifications
Essential criteria
- Registered Nurse
- Evidence of recent development
- Proven ability in teaching and presentation skills
Desirable criteria
- Relevant post registration diabetes course eg “Care of the adult with Diabetes module”
- Recognised Teaching qualification
- Advanced communication skills course or counselling course
Experience
Essential criteria
- Clinical diabetes experience
- Evidence of management/ leadership skills and autonomous practice
- Evidence of continued professional development
- Can demonstrate clinical experience in a variety of areas
- Practical experience of working with people with diabetes and their families / carers
Desirable criteria
- Delivering patient education sessions
- Experience in independent diabetes clinic consultation
- Insulin initiation teaching
Knowledge
Essential criteria
- Effective teaching and presentation skills
- Effective communication skills (both verbally and written)
- Empowerment and counselling skills
- Be able to manage time and workload effectively
- Approachable, honest, enthusiastic and reliable
