Job overview
This is an exciting new role in our Hepatology team to coordinate the surveillance for hepatocellular carcinoma.
The Liver Surveillance Pathway Coordinator will be responsible for supporting the care of patients living with liver cirrhosis through the implementation of a nationally recognised database to identify and track individuals requiring treatments to detect Hepatocellular carcinoma.
This role will enable the hepatology service to continue to deliver exceptional patient care and meet minimal national standards, as outlined in the "NHS England Hepatocellular carcinoma (HCC) surveillance: minimum standards" guidelines.
The Surveillance Pathway Coordinator will support the patient through the journey from identification of cirrhosis and referral to conclusion of their intervention, thus eliminating barriers to care.
The Surveillance Pathway Coordinator will work closely with the wider gastroenterology MDT to ensure a patient focused service and will keep accurate patient and activity records to facilitate reporting back to Commissioners regarding performance and service success.
Main duties of the job
- To support the trust to achieve the national minimum standards of Hepatocellular Carcinoma (HCC) surveillance as outlined in the NHS England Hepatocellular Carcinoma (HCC) surveillance: Minimum
standards. - Supporting the implementation HCC a nationally recognised HCC surveillance database within the trust.
- Work in partnership with the Hepatology surveillance CNS to implement this programme of work within the trust
- Work across the hepatology team as a whole to support workstreams that optimise the care and health of liver patients who access the hepatology service

Detailed job description and main responsibilities
- Monitoring & Managing the HCC surveillance database ensuring it is up to date
- Support the reporting of data to local and national commissioners and the cancer alliances in a robust and timely way.
- Coordinate patient's abdominal ultrasound scans, blood tests and clinical reviews and ensuring results are evaluated by a clinical nurse specialist.
- Liaise with different departments within the trust and externally to the trust to ensure patient care is met.
- To help reduce patient cancellations and non-attendances for tests and clinic appointments influencing rapid surveillance pathway.

- Computer literate with good working knowledge of Microsoft packages
