The Stroke Team at Great Western Hospital is looking for a motivated and experienced Registered Nurse to join our established team of Stroke Specialist Nurses. This is an exciting opportunity to work within a highly skilled, supportive multidisciplinary team delivering urgent and specialist stroke care in a busy district general hospital.
At Great Western Hospital we provide life-saving acute stroke treatments including thrombolysis, anticoagulation reversal, and rapid assessment for mechanical thrombectomies. The role is fast-paced and clinically rewarding, requiring excellent assessment skills and the ability to respond quickly to patients presenting with acute neurological symptoms.
As a Stroke Specialist Nurse, you will be one of the first points of contact for patients experiencing new neurological symptoms, supporting rapid assessment and facilitating timely treatment to ensure the best possible outcomes for patients. You will work closely with the stroke consultant and the wider multidisciplinary team to coordinate care for patients presenting from across Wiltshire.
The Stroke Nurse Service operates 7 days a week from 08:00 to 18:00, providing specialist stroke assessment and management alongside a stroke consultant.
Main duties of the job
- Provide specialist nursing assessment and clinical decision-making for patients presenting with suspected stroke or acute neurological symptoms
- Support the delivery of urgent treatments including thrombolysis, anticoagulation reversal and blood pressure management.
- Assist in the assessment and referral pathway for mechanical thrombectomy
- Work collaboratively with the stroke consultant and multidisciplinary team to ensure rapid diagnosis and treatment
- Coordinate and review referrals from across Wiltshire
- Contribute to service development, audit, and the ongoing improvement of stroke care pathways
- Provide education and support to patients, families, and healthcare professionals

Detailed job description and main responsibilities
1. To be professionally and legally accountable for all aspects of own work.
2. To work within the framework of local and National Clinical Guidelines for Stroke and NICE clinical guidelines 68, including future updates.
3. Be an autonomous practitioner (i.e. be responsible for own assessment, treatment and discharge decisions; within own level of competence, professional and service parameters.)
4. Use high levels of insight, skill and experience to carry out clinical role.
5. Implement the TIA Care Pathway to patients referred to the stroke service, triage all TIA referrals from a variety of sources e.g. GP, Clinic, Ward, A&E and order and coordinate investigations and clinic appointments in a timely manner within specified target times of 24hrs or 7 days.
6. Undertake similar assessments in working environments other than the clinic e.g. the ED department, meeting the 4 hour wait target.
7. In conjunction with the medical team the post holder may have responsibility for reviewing results and ensuring appropriate action taken as required.
8. Deliver information and advice to clinic patients about secondary prevention of stroke and lifestyle choices which may be contentious information.

Qualifications / Experience / Skills
Essential criteria
- First level NMC registered nurse
- Qualified to degree level or equivalent training and experience
- Further related training to stroke or neurosciences (e.g. ENB 148 or other recognised training)
- Significant experience working within a relevant specialist area e.g. stroke unit, at a minimum of band 5 level
- Experience of interdisciplinary working
- Experience of clinical audit
- Knowledge of current best practice, National Clinical Guidelines for Stroke and government related documents e.g. National Stroke Strategy, NICE guidance for acute stroke and TIA management
Desirable criteria
- Teaching qualification / training (e.g. ENB 998 / TLHP or other recognised training)
- Nurse Prescribing qualification
- Experience of leading change management
- Demonstrates ability and knowledge on how to undertake audit and feedback to clinicians
- Able to contribute to the development, implementation and monitoring of local protocols and objectives
