Based in Acton House, Worcester or St Owen's, Hereford.
Following expansion of Children’s Continuing Care at HWICB we are seeking an enthusiastic and self-motivated Registered Children’s Nurse to join our team. Roles and responsibilities will include the completion of complex health assessments and reviews for children / young people, chairing multi-agency assessment meetings to inform the decision-making process and transition planning and preparation- all within the context of the NHS National Continuing Care Framework (Children and Young People).
This role is an integral part of the Children’s Continuing Care (CCC) Team, which sits alongside the Continuing Healthcare (CHC) Team and is part of NHS Herefordshire and Worcestershire Integrated Care Board (HWICB), which is the NHS commissioning organisation working across Herefordshire and Worcestershire.
Main duties of the job
You will work with children/families, multi-agencies and other professionals, across all sectors- ensuring a ‘Team around the child’ ethos is developed and maintained for those children / young people who require Continuing Care support and/ or assessment within Herefordshire and Worcestershire.
You will have excellent communication skills and be able to work efficiently, prioritising work to meet multiple deadlines, collate/report data timely and appropriately, and maintain standards relating to administrative processes in order to ensure the delivery of a high quality, responsive service to meet the health needs of the caseload.
You will be able to work confidently both face to face and on a digital platform, producing clear and concise written assessments and reports- which are accurate and evidence-based.
You will be registered with the Nursing and Midwifery Council as a Registered Children’s Nurse RN Child/ RSCN and with appropriate experience in Community Children’s Nursing and/or Children’s Continuing Care.

Essential criteria
- Current registration with the Nursing and Midwifery Council (NMC): RN Child/ RSCN/SCPHN (Health Visiting/School Nursing), LD/MH Nurse with extensive experience of managing a caseload of patients.
- Educated to degree level or equivalent experience in Healthcare service
- Experience of working with patients with multiple/complex care
- Evidence of working extensively with children and young people within their speciality field
- Evidence of assessment and reviewing skills
- Experience of delivering/supporting training
Desirable criteria
- Experience of working within the Continuing Care arena
- An understanding of local health and social care commissioning
- Evidence of participation in using recognised assessment tools
