This is an exciting opportunity to work at the heart of a truly integrated service, bringing together health, social care and community partners to support people with complex needs following hospital discharge.

Working as part of a multi-disciplinary team, you will undertake holistic assessments, develop person-centered management plans and coordinate care for individuals with complex health and social care needs.

You will work closely with GPs, social care teams, acute hospitals, care homes, voluntary organisations and other system partners to ensure seamless, integrated support for patients across the community.

This role requires excellent clinical reasoning, strong organisational skills and the ability to work autonomously while contributing positively to team leadership and service development.

If you are passionate about integrated care, supporting safe hospital discharge and helping people live independently within their communities, we would love to hear from you.

Main duties of the job

  • Undertake comprehensive holistic assessments for people with complex and long-term needs.
  • Coordinate and manage a caseload of patients following hospital discharge
  • Develop and review person-centered care and support plans.
  • Support safe and effective discharge planning using a 'Home First' approach.
  • Work collaboratively with health, social care and voluntary sector partners.
  • Facilitate access to appropriate services, equipment and community resources.
  • Provide advice and support relating to safeguarding, medication and long-term care needs.
  • Participate in multidisciplinary team meetings, care home MDTs and system partner forums.
  • Contribute to service improvement, quality initiatives and audit activities.
  • Support and develop colleagues, students and junior team members.
  • Assist with team coordination, workload allocation and capacity management.
