Job overview

We are looking for a clinical pharmacist who is passionate about Frailty and supporting patients to regain independence with their medication.

You will be working closely with a pharmacy technician, nursing staff, therapists, ACPs and GPs on a day to day basis in an inpatient setting and must demonstrate good communication skills.  

The pharmacist will predominantly support a 30 bedded inpatient rehabilitation unit in Yate (minimum 15 hours per week), but may also provide cover to one of our other units in Hengrove, Thornbury or Clevedon. There would also be an opportunity to work with one of our community based teams in Bristol. Preferences can be discussed with the service manager prior to or at interview.

It would be possible to utilise your prescribing qualification (if achieved) or work within the organisations Enabling policy once competent. 

This position is currently fixed-term for 12 months to provide cover for a pharmacist that has recently taken up a secondment in a senior post elsewhere in the organisation. There is a possibility this post could become permanent, but this would not be confirmed until the second half of the fixed-term. Full-time hours may also be possible, depending on the outcome of recruitment to other vacant posts. 

Main duties of the job

  • Medicines reconciliation on admission and clinical screening of discharge summaries. 

  • Reviewing blood results/physical observations and making appropriate recommendations/adjustments to prescribed medication.

  • Conducting patient assessments and identifying interventions that will support independence with medication and facilitate timely discharge. 

  • Provide specialist advice for patients with complex medication regimens (including deprescribing) and those with high-risk medicines.

  • Work confidently as part of a multidisciplinary team and support prescribers.

  • Provide evidence-based medicines information and advice to health professionals.

  • Carry out audit and contribute to maintaining governance arrangements (e.g. policies and procedures)

Qualifications

Essential criteria
  • Pharmacy degree (MPharm or equivalent) plus 1 year pre-registration/foundation training.
  • Registered as a practising pharmacist with the General Pharmaceutical Council (GPhC).
Desirable criteria
  • Post-graduate qualification in clinical pharmacy, therapeutics or pharmacy practice, or equivalent experience.
  • Independent prescriber or willingness to undertake training

Experience

Essential criteria
  • Post-registration experience in a clinical setting/relevant role
  • Experience of working with a multidisciplinary team.
  • Experience of undertaking medication reviews.

Skills

Essential criteria
  • Excellent communication and negotiation skills with service users/carers and health care professionals, including written, verbal and electronic.
  • Excellent organisation and time management skills and ability to prioritise workload according to urgency and importance.
  • Builds effective working relationships with other health care professionals and outside agencies.
  • Flexible approach to different working environments.
