Job overview
An exciting opportunity has arisen for a Registered Nursing Associate to join the South West Older Adult Community Team. The applicant must have completed the Nursing Associate foundation degree and be a registered Nurse Associate on the NMC register. This post is not offering the training to gain this qualification.
The successful applicant will work as part of the busy community team offering an identified pathway to Service Users and act as a keyworker for a defined caseload. You will support with both medicines management and psychological approaches in your delivery of interventions to meet individual's needs. You will work collaboratively with patients and their families in order to support with goal setting and meeting identified need.
The successful applicant will be able to identify which base they are aligned to with the team covering two sites. However, their clinical demands and caseload will cover both the South and West of the team's clinical work.
Main duties of the job
The successful applicant will work as part of the busy integrated community mental health team offering a recovery focused and living well interventions to service users. The applicant will manage a defined caseload acting as keyworker with supervision and support from the wider multi-disciplinary team. This role will involve multiagency working and liaison with other services within Tees Esk and Wear Valley NHS Trust.
Following initial patient assessment completed by the Registered Nurse you will be responsible for implementing care plans, including providing information and advice to patients, clients and carers. In collaboration with the Registered Nurse evaluate and adjust programmes of care in partnership with patients, clients and carers and the multidisciplinary team.
You will promote health and provide and monitor care with all patients and carers. Delivering interventions to a high standard in line with the Trust governance framework.
The role requires you to respond to enquiries from relatives and carers, dealing with concerns and actively and problem solve to improve the patients experience, escalating to the registered nurse as required.
Accurately undertake risk assessments, using contemporary assessment tools to improve safety and quality of care alongside the registered nurse

Essential criteria
- Registered Nursing Associate on the NMC register
- Nursing Associate Foundation Degree qualification
- GCSE Grade A-C in Maths & English or skills level 2 Maths & English or equivalent
Desirable criteria
- Qualification in assessment, supervision and teaching in clinical practice
Experience
Essential criteria
- Up to date understanding of the scope of and role of the Nursing Associate in context of the nursing and multidisciplinary team and the organisation, and how the role may contribute to service development
- Understands and acts in line with NMC professional standards for practice contained within The Code.
- Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice
- Experience in a health care setting
