You will be working as part of the Stroke Therapy Team. The team is made up of Physiotherapists, Occupational therapists, Speech and language therapists, Assistant practitioners and a Team administrator, all of whom are dedicated and passionate to provide outstanding care to stroke survivors. The successful applicant will also be passionate about Stroke and have experience working in health and social care, experience working with patients and skills and behaviours suitable for the post.
The role when working across both acute and community pathways, is to support the assessment and treatment of stroke survivors, to assist with
discharge planning, family/carer communication, attending daily/weekly MDTs, providing goal-led rehabilitation and liaison with the wider MDT.
You will be a motivated individual, keen to implement change and empower staff to be the best they can be.

Our Furness Stroke Therapy Team currently provides a rotation between acute inpatient Stroke and community Stroke rehabilitation – allowing staff to be a part of the whole process from admission to discharge and ongoing rehabilitation in the community. The Furness team covers an extensive area including Furness, Millom and South Lakes. The service is currently under development and will include Saturday and eventually Sunday working. You will require access to a car.
The stroke team are supportive and committed to provide a friendly and flexible attitude which is patient focused and dedicated to quality care.
Our aim is to ensure all our patients have access to appropriate treatment in the right location at the right time. Our approach to stroke services will require a degree of flexibility as well as creative thinking to establish and form new pathways of care. You will be providing care across the pathway within the acute (ward) and community settings (patients home/other settings/clinics).

We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

The post holder will work as a member of the ICST to provide
assessment and rehabilitation as a part of the wider multi-disciplinary service.
Provide assessment and treatment to patients with diverse health
needs, mainly within their own homes but also in a variety of
settings under the supervision of a registered practitioner.

You will either already have an Assistant Practitioner Qualification for this role or be expected to work towards this via UCLAN via an apprenticeship. If you need to work towards this you will be positioned at band 3 until the completion of your qualification.
